BACKGROUND: Cellular apoptosis plays a key role in intestinal ischemia-reperfusion injury and is closely related to the expression level of apoptosis-inhibiting Bcl-2 and apoptosis-promoting Bax.OBJECTIVE: To probe into the effect of sub-hypothermia ischemic pretreatment on the expression of apoptosis-related Bcl-2 and Bax protein after intestinal ischemia-reperfusion in rats.DESIGN: Randomized controlled trial.SETTING: Department of Surgery and Department of Biochemistry, Medical School of Xianning College of Hubei Province.MATERIALS: The experiment was conducted in the Central Laboratory of the Medical School of Xianning College from April to December 2002. Totally 32 male healthy Wistar rats were used. The animals were fasted for 24 hours before operation with free access to water. Then they were randomly divided into sham-operation control group, ischemia-reperfusion group, ischemia-pretreatment group, and sub-hypothermia pre-treatment group with 8 rats in each group.METHODS: Ischemia-reperfusion model was established in all the groups except sham-operation group. ① Superior mesenteric artery was exposed for 2 hours without occlusion in the rats of sham-operation group the material was selected after the experiment. Superior mesenteric artery in ischemia-reperfusion group was occluded for 60 minutes the material was selected after stopping occlusion for another 60 minutes. Superior mesenteric artery in ischemia-pretreatment group was occluded for 5 minutes and stopped occlusion for 5 minutes, and the other procedure was the same as that in ischemia-reperfusion group. In sub-hypothermia pretreatment group, pieces of fractional ice filled around the small intestine for sub-hypothermia (33-35 ℃) before performing ischemia pretreatment, then followed by the same procedure as that in ischemia-pretreatment group. ② The middle part (4 cm) of the rats' small intestine was chosen in each group. Each small intestine was divided into 2 segments, which were put in formaldehyde and glutaric dialdehyde separately for fixing to prepare electron microscopic samples. Absorbance value in each group was measured with image analysis under the microscope after immunohistochemical staining. Ten fields in each group were chosen for measurement to calculate the average value. Intestinal mucous membrane injury was graded according to Chiu's criteria. Degree 0: normal mucous membrane villus degree I: lower interspace of epithelia expanded, with capillary congestion usually appearing at the tip of the villus degree Ⅱ: lower interspace of epithelia expanded, accompanied with moderate separation of epithelial lining from lamina propria degree Ⅲ: large amounts of epithelial lining at the both sides of villus separated from lamina propria and the tips of some villi were injured degree Ⅳ: the villus was injured accompanied with capillary of lamina propria exposed. Cellular ingredients of lamina propria could be seen increased degree Ⅴ: lamina propria was destroyed and incomplete with bleeding and ulcer.MAIN OUTCOME MEASURES: ① Change of absorbance of Bcl-2 and Bax protein after intestinal ischemia-perfusion in each group ② grading of intestinal mucous membrane in each group ③ histological change of intestinal mucous membrane in each group.RESULTS: The thirty-two rats entered the stage of results analysis with no loss in the midway. ① Change of absorbance of Bcl-2 and Bax protein after intestinal ischemia-reperfusion in each group: It was obviously increased in ischemia-reperfusion group compared to that of sham-operation group (4.03±1.02, 9.56±1.32, P < 0.01 5.67±1.34, 19.07±1.63, P < 0.01). The expression of Bcl-2 in ischemia-pretreatment group was increased compared with that of ischemia-reperfusion group (9.56±1.32, 15.03±1.44, P < 0.01), while the expression of Bax was decreased (19.07±1.63, 14.11±1.21, P < 0.01). Expression of Bcl-2 in sub-hypothermia group was slightly increased when compared to ischemia-pretreatment group (15.03±1.44, 18.17±2.03, P < 0.05), while the expression of Bax was decreased (14.11 ±1.21, 11.58±1.04, P < 0.05). ② Grading of intestinal mucous membrane injury in each group: The intestinal mucous membrane in sham-operation control group was basically normal with degree 0 injury in ischemia-reperfusion group, there were 2 cases of degree Ⅱ injury of intestinal mucous membrane, 3 cases of degree Ⅲand 3 of degree Ⅳ, which significantly outnumbered those in sham-operation control group (P < 0.01). In ischemia-pretreatment group, there were 2 cases of degree Ⅰ injury, 4 of degree Ⅱ and 2 of degree Ⅲ, which were significantly fewer than those in ischemia-reperfusion group (P < 0.01). In sub-hypothermia pretreatment group, there was 1 case of degree 0 injury of intestinal mucous membrane, 3 of degree Ⅰ injury and 4 of degree Ⅱ, which were fewer than those in ischemia pretreatment group (P < 0.05). ③ Results of electron microscopic observation on the histological morphology of intestinal mucous membrane in each group: In sham-operation control group, microvilli of the epithelial mucous membrane were sparse, shortened and shedded. Mitochondria were swollen obviously and vacuoles were denaturated endoplasmic reticulum was arranged in disorder and swollen. In ischemia-pretreatment group, microvilli of the epithelia at mucous membrane were practically orderly. Endoplasmic reticulum of the mitochondria was slightly swollen. In sub-hypothermia pretreatment group, microvilli of the epithelia at mucous membrane were arranged orderly, and the endoplasmic reticulum of the mitochondria was not swollen obviously.CONCLUSION: Performing ischemia pretreatment before intestinal ischemia-reperfusion can upregulate the expression of Bcl-2 protein and downregulate the expression of Bax protein, thus inhibiting cellular apoptosis induced by intestinal ischemia-reperfusion so as to protect from ischemia-reperfusion intestinal injury. The state of sub-hypothermia can strengthen the protective effect of ischemia pretreatment.
