目的 研究立体定向联合毁损杏仁核内侧核群和扣带回前区对Ⅰ型精神分裂症患者脑脊液内单胺类递质的影响 ,探讨手术影响单胺类递质水平及手术治疗精神分裂症的机理。方法 用立体定向技术射频毁损Ⅰ型精神分裂症患者杏仁核内侧核群和扣带回前区 ,用高效液相色谱法 (HLPC)测定手术前后患者脑脊液DA、5 HT、NE的含量 ,对含量的变化进行统计学处理。结果 治疗前不同性别、年龄及病程之间DA、5 HT、NE未见明显差异(P >0 .0 5 ) ;治疗前DA明显高于对照组 ,治疗后DA下降 ,与手术前相比有显著性差异 (P <0 .0 5 ) ;治疗前 5 HT和NE明显低于对照组 ,治疗后升高 ,但仍低于对照组 (P >0 .0 5 )。结论 毁损杏仁核内侧核群和扣带回前区可以使Ⅰ型精神分裂症患者脑脊液内DA水平升高 ,5 HT和NE水平降低 ,这一变化提示调节递质水平可能是手术治疗精神分裂症的途径之一
