Objective To evaluate the preliminary result of clinical application in the repair of coracoclavicular ligament injury by artificial tendon produced with tissue engineering. Methods Tendon cells were isolated from 12- week induced labor embryo donated from a healthy woman with selective termination of pregnancy with informed consent. 5× 106/ ml tendon cells were co- cultured with composite scaffold materials of carbon fibers and polyglycolic acid (PGA) for 5 to 7 days, and the artificial tendons were then used...

Methods Tendon cells were isolated from 12- week induced labor embryo donated from a healthy woman with selective termination of pregnancy with informed consent. 5× 106/ ml tendon cells were co- cultured with composite scaffold materials of carbon fibers and polyglycolic acid (PGA) for 5 to 7 days, and the artificial tendons were then used to repair 5 cases with degreeⅡ andⅢ distal clavicular fractures and degreeⅢ dislocation of acromioclavicular joint. All the cases were followed up for 7 to 13 months, In two cases during the time of removal of the internal fixation after 3 months and 6 months respectively, micro- tissues were sampled, and the tissues were observed by histological examination and short tandem repeated loci examination. Results The shoulder function of all cases recovered well with first intention, and no local or systemic immunological rejection were observed. Histological examination showed that the tendon cells were spindle- like or slight strip- like, no obvious lymphocytes infiltration was observed and the tissue fibers arranged uniformly and regularly with capillary hyperplasia. The electrophoresis typing of D3S1754, Cyar04 and TH01 gene loci showed that the tissue engineered tendon survived in vivo. Conclusion Coracoclavicular ligament injury repaired by tissue engineered tendon is a feasible method. The allogeneous tendon cells can survived in vivo at least 6 months without obvious rejection.

目的总结组织工程化人工肌腱修复喙锁韧带的早期临床效果。方法用自愿中止妊娠健康妇女捐赠的 12周引产健康胚胎肌腱分离培养肌腱细胞,以 5× 106/ ml细胞密度种植在碳纤维和聚羟基乙酸的复合支架材料上,体外培养 5~ 7 d后植入体内,修复 5例锁骨外端Ⅱ、Ⅲ度骨折和肩锁关节Ⅲ度脱位伴喙锁韧带损伤。全部病例随访 7~ 13个月,平均 9.3个月。其中 2例分别于术后 3个月、 6个月行内固定取出术时,取微量组织行组织学检查及短串联重复位点检测。结果全部病例早期均无局部及全身反应,伤口一期愈合,上肢功能恢复正常,可从事原工作。组织学检查证实无明显淋巴细胞浸润,细胞呈梭形,纤维组织排列整齐、均匀,可见毛细血管增生。用法医物证技术检测 D3S1754、 Cyar04, TH01基因座,发现 2例均存在非自体组织等位基因,表明植入细胞保持了活力。结论用组织工程学原理构建的人工肌腱修复喙锁韧带损伤是可行的手术方法,植入的同种异体细胞至少在 6个月内保存了活力,无明显排斥反应。

Objective:To probe into the treatment of acromio clavicular dislocation and clavicular fracture.Methods:Eleven cases of acromiocl avicular dislocation,7 cases of distal clavicular fracture and 4 cases of middle clavicular fracture were treated with hook plates in recent 2 years.Twenty case s were followed-up.The follow-up periods lasted from 4 to 18 months with an average of 10.2 months.Results:Of the 20 follow-up cases,14 were excellent and 6 satisfactory according to the Lazzcano standard There were no other complica tions.All the fractures were healed and no redislocation was observed.Conclusion :Hook plates show the advantage of rigid fixation,no invasion into article surfa ce,and capable of early exercises in the treatment of acromioclavicular dislocat ion and clavicular fracture.
