Fifteen cases of unstable thoracic and lumbar spinal fractures with paraplegia were treated with Harrington distraction rods combined with cross wiring. The results were quite satisfactory. There was no loosening of the Harrington rod after operation. Firm fixation of the spine made nursing care easier and complications of paraplegia reduced after operation. Restoration of normal anatomic alignment of the spine might be beneficial for possible recovery of spinal cord injury and the technique is easy...

Eighteen cases of instable thoracolumbar spinal fracture complicated by paraplegia were treated with Harrington distraction rod. Follow-up for 3 to 20 months revealed that the neurological function in the 7 patients with incomplete paraplegia became almost normal,but among the 11 patients with complete paraplegia only 2 improved in sensation and motion. 报告了18例Harrington撑开棒治疗脊柱胸腰段不稳定性骨折伴截瘫的临床效果。随访3~20月,7例不完全性截瘫基本恢复正常;11例完全性截瘫2例部分恢复。认为撑开棒具有牵引和固定的双重作用,能使骨折解剖复位,解除神经压迫,并长期维持脊柱的稳定性,是一种操作简便、疗效确切的治疗方法。同时介绍了钢丝固定损伤椎骨棘突和两侧撑开棒的方法。

取9名手术病人的腰椎间盘纤维环和髓核组织,作扫描电镜观察。观察结果,髓核的胶原纤维排列呈不规则的网状结构、胶原纤丝间有较大的空隙。胶原纤丝直径在0.1~0.2μm。胶原纤丝上附有直径0.003~0.02μm的颗粒分子。胶原纤丝示有特异性周期的带状结构。纤维环呈分层状排列整齐紧密的胶原纤维结构。胶原纤维层间有少许胶原纤丝相连。胶原纤维分层厚为1.77~3.22μm。纤维环胶原纤丝直径与髓核的相同。正常椎间盘与腰椎间盘突出症的扫描电镜观察,未见有形态上的差异。
