The anatomic reduction rates of thoracic and lumbar spinal fractures were increased by Edwards devices, but the stresses on Harrington rods were risen linely. The point on Harrington rod where the rod-sleeve had been affected severely when the horizontal forces were increased, but the general curve characteristic of stress distribution were kept unchanged. The ratchet-shaft junction was the stress riser because of stress concentration. The stresses on anterior border of Harrinton rod were higher than on the posterior border. The stresses on ratchet part were much more than on smooth part. The results recommended that the rod-sleeve should be placed to the stress riser as fartther as possible, or the stress riser should be as closer to the upper hooks as possible in clinics in order to reduce the risk of mechanical failure of Harrington rods.

使用Edwards套棒可提高Harrington装置对脊柱骨折的解剖复位率,但同时使Harrington棒各点应力值呈线性增加,尤以套棒所在点受影响最大,然而总体应力分布趋势不变,棒体——齿棘区交界处应力集中为棒的危险断面,棒前侧应力值高于后侧;齿棘部高于棒体部。实验结果提示,临床应用Ebwards套捧时应注意将其安置在尽量远离危险断面处,并使危险断面尽量靠近上钩,以减少在交变应力作用下应力集中造成的Harrington棒疲劳断裂。

Posterior lumbosacral fusion using bone graft without internal fixation requires long immobilization and has high rate of pseudoarthrosis,To overcome such limitation technique of spinal internal fixation has been developed and applied in past twenty years.Since 1982,the authors have done lumbosacral fusion with internal fixation and bone graft by using Harrington alar hook(32 cases),luque ring(26 cases),Galveston technique(7 cases ),Steffee plate(10 cases),Dick pedicle screw(1 cases),RF instrumentation(3 cases)and CD technique(4 cases).Totaly 83 cases have been treated ,which include paraletic pelvic tilt(9 cases),spinal fracture(4 cases),lumbosacral tuberculosis(5 cases),degenerative sdenosis(21 cases);L5-S1 spondylolithesis(38 cases),L5-S1 disc protrusion(4 cases );L5 semivertebra(1 cases) and neurofibromatosis(1 cases).Patients have been follow-up for 63.7months on average. Good fusion has been seen in 82 cases(98.8%).The procedure does not need long immobilization and has less complication.

自1982年以来,应用Harrington骶骨钩法(32例)、Luque环法(26例)、Galveston法(7例)、Steffee法(10例)、Dick法(1例)、RF法(3例)及CD法(4例)等行腰骶部内固定加植骨融合治疗各种病患83例,其中麻痹性骨盆倾斜9例,脊柱骨折4例,腰骶结核5例,退行性椎管狭窄21例,各种原因L_5-S_1脊椎滑脱38例,L_5-S_1椎间盘突出4例,L_5半椎体及神经纤维瘤病各1例。平均随访63.7月,一期获得满意后融合者82例(占98.9%),术后不需长期卧床,并发症较少。

Objective:To further investigate CT diagnosis of spinal fracture so that it provides the basis for clinical treatment.Methods:X-ray and CT characteristic findings of spinal fracture were funther analyzed.There were 65 males and 19 females,ranged in age from 17~69 years.Results:The location and types of spinal fracture,spinal three columns structure,narrowing of spinal canal,the areas of cross section of spinal canal,the nerve roots and spinal cord to be compressed by bone segments were exactly revealed on CT.Conclusion:The surgical indications of spinal traumas are decided according to the information of CT scans.CT is first selective mean for spind fracture.

目的:为进一步认识脊柱骨折的CT影像诊断,为临床治疗提供依据。方法:对84例脊柱外伤病例进行临床、X线片及CT分析。男性65例,女性19例,年龄17~69岁。结果:CT检查能准确揭示其骨折部位,骨折类型,脊柱三柱结构,特别是椎管狭窄及椎管横截面积与骨片对神经根和脊髓的压迫影像。结论:通过CT影像来判断手术治疗的适应证,同时证明CT检查为重要谦宜的首选方法
