METHODS: A randomized double blind parallel controlled clinical trial was per formed.A total of 41 hospitalized patients,who needed energy supply,were randoml y divided into trial group(n=20) and control group(n=21). They received 50 g/L D - fructose or 50 g/L glucose 500 mL/L intraveneouly daily for 3 days.The blood glucose concentrations were estimated before and 0 hour and 2 hours after each a dministration.Liver and renal functions were also tested. RESULTS:Blood glucose level was (1.8± 1.5)mmol/L on everage soon after injec ton of 50 g/L D- fructose,and it was obviously less than that of 21 patients (7 .2± 2.2)mmol/L after injection of 50 g/L glucose.Two hours after injection,bloo d glucose levels in trial and control group were (5.5± 1.6)and (6.3± 2.4)mmol/ L repectively.The difference between them had no statistical significance.Compar ative results of blood glucose level before injection and 2 hours after injectio n showed that patients in trial group had no statistical significance;while pati ents in control group had. CONCLUSION: Blood glucose fluctuation after injecting 50 g/L D- fructose par enteral solution is obviously less than 50 g/L glucose does.5 g/L D- fructose w as efficient and safe for energy supply in hospitalized patients.

Objective To study the effect of glucose change on expression of prepro orexin and its receptors, orexin 1 receptor (OX 1R) and orexin 2 receptor (OX 2R) in rat hypothalamus tissue and pancreatic islets cells. Methods The acute hypoglycemia rat model was induced by subcutaneous injection of insulin. The effect of 2 8 mmol/L and 8 3mmol/L glucose on pancreatic islet cell orexin system was detected in pancreas islet cell culture. The expression of prepro orexin and OXR mRNA was examined by reverse transcription polymerase chain reaction in rat hypothalamus tissue and pancreatic islets cell. Results Expression of orexin mRNA increased about 150% in insulin induced hypoglycemia rat hypothalamus tissue, compared to control group ( P <0 01), whereas expression of OX 1R mRNA decreased up to 30% ( P <0 01). However, expression of OX 2R mRNA did not show significant change in comparison with control group. The expression of prepro orexin mRNA increased up to 100% in rat pancreas islet cell cultured in 1640 medium with 2 8 mmol/L glucose, compared to 8 3mmol/L glucose after 6 h ( P <0 01). But the expression of OX 1R mRNA level was not sensitive to acute glucose fluctuation. Conclusions The orexin and OX 1R are stimulated by falls in plasma glucose and inhibited by signals related to feeding. Moreover, glucose plays a role in modulating the gene expression of prepro orexin in rat pancreatic islet cells.

Objective To compare the therapeutic efficacy and safety between the short term continuous subcutaneous insulin infusion (CSII), and insulin multiple daily injection (MDI) in treatment of elderly patient with type 2 diabetes who were poorly controlled. Methods Fifty eight elderly diabetic patients were randomly divided into CSII group and MDI group. The short acting Novolin R was subcutaneously infused using pump continuously in CSII group, and Novolin R was subcutaneously injected before three meals a day and medium acting Novolin N was injected before sleep in MDI group. The glucose level, therapeutic time, glucose fluctuation, insulin dosage, incidence of hepoglycemia were observed in the first month, and at the end of third month, the insulin dosage and HbA 1c were observed. Results In the first month of therapy, the glucose level, therapeutic time〔(7 3±3 6)d vs(12 1±5 8)d〕,glucose fluctuation 〔(4 4±1 6)mmol/L vs (6 9±2 7)mmol/L〕, insulin dosage 〔(33 7±6 3)U/d vs (42 8±7 9)U/d〕, and incidence of hepoglycemia (2 times vs 14 times)of CSII group were better than MDI group. The insulin dosage and HbA 1c of CSII group were significantly lower MDI group after 3 months. Conclusions The efficacy and safety of short term insulin pump infusion are better than multiple daily injection in intensive therapy of senile diabetes, and the efficacy last longer to medium or long term.

目的 对比短期应用胰岛素泵与多次皮下注射胰岛素对血糖控制不佳的老年 2型糖尿病患者的治疗效果及安全性。 方法 老年 2型糖尿病患者 5 8例 ,分为胰岛素泵治疗组和胰岛素多次皮下注射治疗组 ,泵治疗组给予胰岛素泵持续皮下注射短效胰岛素加餐前大剂量 ;注射治疗组 3餐前注射短效胰岛素和睡前注射中效胰岛素。泵治疗组治疗 1个月后换用多次皮下注射方法继续治疗。对比两组血糖水平、血糖达标时间、血糖波动值、胰岛素用量、低血糖发生率以及治疗 3个月后糖化血红蛋白和胰岛素用量。 结果 在第 1个月内 ,泵治疗组和注射治疗组血糖达标时间〔( 7 3±3 6 )d和 ( 12 1± 5 8)d〕、血糖波动值〔( 4 4± 1 6 )mmol/L和 ( 6 9± 2 7)mmol/L、胰岛素用量〔( 33 7± 6 3)U/d和 ( 4 2 8± 7 9)U/d〕、低血糖发生率 ( 2次和 14次 )均有明显差别 (P <0 0 1及P <0 0 5 ) ;3个月后糖化血红蛋白和胰岛素用量差异仍有显著性。 结论 短期应用胰岛素泵强化治疗能更快、更有效、更安全的控制血糖 ,并且具有较好的中长期效果。
