Objective:To determine the timing of dist ractor's removal after the prolonging of the rabbits' mandibles by osteogenesis distraction through the study of new- generated bone retraction.Method: In this study we use 16 adult rabbits(provided by Experimental Animal Department of Fourth Military Medical University), weigh ing from 2.4 to 2.6 kg, randomly divided into 4 groups and each group 4. For eve r y rabbit we apply mandibular osteotomy, and then introduce internal distractors through which both the two sides of the rabbits' mandibles can be prolonged. Lat ency period is 7 days, distraction period is 10 days and retention period is res p ectively 2, 4, 8, 12 weeks. Meanwhile we carry out second operation to remove th e distractors and at the same time titanic nails are implanted through which we c an measure the length of the new bone of the mandible.Result: After 8 weeks the new bones with retention period of 2 or 4 weeks significantly retract, 2.1±1.2 mm, 1.4±1.3 mm respectively, and those with retention period of 8 or 12 weeks no t.Conclusion:The timing of removing the distractors applied in r abbits' mandibe is 8 weeks after the completion of distraction;if the distractor s are removed too early,the new bones may generate significant retraction.This m ay give some suggestion to clinical operations. 目的 :研究家兔下颌骨延长术后新生骨回缩情况 ,确定牵张器的拆除时机。方法 :成年家兔 16只 ,体重 2 .4~ 2 .6kg ,随机等分为 4组 ,每组 4只 ;行双侧下颌骨骨切开术 ,采用内置式牵张器对双侧下颌骨同时进行牵引。延迟期 :7d ;牵张期 :10d ;牵引速率 0 .5mm/次 ,2次 /d ;保持期分别为 2周、4周、8周、12周。保持期后行第二次手术取出牵张器 ,并同时植入钛钉为参照 ,判断下颌骨改建后新成骨有无缩短 ,观察期为 8周。结果 :保持期为 2周、4周组的实验动物新生骨在 8周后明显回缩 ,分别为 2 .1± 1.2mm、1.4± 1.3mm ;而保持期 8周、12周组 ,8周后新生骨未见明显回缩。结论 :兔下颌骨牵张延长术后 ,牵张器应在保持期 8周拆除 ;拆除过早 ,新生骨区产生明显回缩。

Objective To investigate the outcome, methods and healing time for reconstructing alveolar cleft. Methods The literatures describing the reconstruction of alveolar cleft were extensively reviewed. The outcome, methods and healing timing of reconstructing alveolar cleft as reported in literatures were summ arized. Advantages and disadvantages of different reconstructive methods and healing time were evaluated. Results The outcomes for reconstructing alveolar clefts included: elimination of oronasal fistulae; stabilizing the maxillary arch; providing a foundation for tooth eruption; providing support for alar base and upper lip. The methods included: bone grafting; osteogenesis distraction; application of tissue-engineered bone and growth factors; guided bone regeneration. The interval between patients ages of 9 and 12 years during the period of mixed dentition was the best time for treatment. Conclusion Iliac cancellous bone grafting performed between the ages of 9 and 12 years during the period of mixed dentition is the gold standard for the reconstruction of alveolar cleft. Osteogenesis distraction and tissue-engineered bone as well as guided bone regeneration are new approaches for the reconstruction of alveolar cleft.

目的探讨齿槽裂修复治疗的目的、方法以及治疗时机的选择。方法查阅1950年至2006年有关齿槽裂修复的文献,归纳文献中报道的不同方法,并评价其各自的优缺点。结果齿槽裂修复的主要目的:关闭口鼻瘘;建立稳定、连续的上颌骨牙弓;为牙齿萌出提供基础;为上唇和鼻底提供稳定支架。主要治疗方法:植骨术;牵引成骨技术;组织工程骨和生长因子应用;引导骨再生技术。患者最佳的手术治疗时机是9~11岁时混合牙列期。结论在9~11岁混合牙列期手术,以髂骨松质骨为移植材料被认为是修复齿槽裂的主要手段。牵引成骨技术、组织工程技术和引导骨再生技术,将是齿槽裂修复的新方向。
