Objective To investigate the effect of filtrating bacteria on breathing circuit after general anesthesia.Method Sixty adult paˉtients under general anesthesia were randomly divided into test group and control group.Respiratory filter were given between tracheal tuba and breathing circuit after intubation in test group.The sample of colony which were cultured with agar plate method were collected in tracheal tuba and breathing circuit before and after operation.Results With the respiratory filter,the breathing circuit almost have no bacteria colonized(P<0.01).Conclusion The respiratory filter not only can filtrate bacteria effectively,but also can control the lower respiratory tract infection obviously and can protect the anesthesia apparatus.

目的观察呼吸过滤器对麻醉呼吸回路的滤菌作用。方法择期全麻手术患者60例 ,随机分为2组 ,实验组的气管导管与呼吸回路之间放置呼吸过滤器。于麻醉前、麻醉结束时对气管导管末端、呼吸回路气管端采样作细菌培养计数 ,并对手术室空气采样行细菌培养。结果麻醉结束后对照组呼吸回路中有大量细菌存在 ,而实验组无细菌发现(P<0.01)。结论呼吸过滤器能有效虑过患者呼吸道及呼吸回路中细菌 ,减少患者医源性肺部感染

Objective To determine the effect of Respiratory filters on clinical measurements of ventilatory function.Methods Random Cross-control Trial was used.Fifty randomly selected subjects referred to our laboratory for routine functional evaluation were performed Forced Vital Capacity tests to compare the difference between with filters and without.Results The addition of Cosmed filters to the pulmonary function testing equiptment caused significant decreases in FVC((-75.42)±(134.20)ml,P=(0.011))、FEV_1((-66.67)±(99.11)ml,P=(0.003)) and PEF((-260.42)±(319.76)ml/s,P=(0.001)),and significant decreases in FVC((-72.31)±(157.84)ml,P=(0.028))、FEF_(200-1200)((-193.08)±(371.5)ml/s,P=(0.014)) and PEF((-425.38)±(488.81)ml/s,P=(0.0001)) were caused with Microgard filters.Both filters did not affect other indices of lung function.There is no significant difference between the impacts of Cosmed and Microgard filters on ventilatory function.Conclusions The Cosmed and Microgard filters caused significant reductions in expiratory flow and volume measurements,however,the changes were within the intraindividual variability of repeated measurements for the spirometric indices and no appreciable classification error was found in diagnosis and interpretation of pulmonary ventilatory function.

目的探讨呼吸过滤器对肺通气功能检查结果的影响。方法采用随机交叉对照的方法,对50例常规功能检查的受试者进行试验,比较应用与不应用呼吸过滤器测定肺通气功能的差异。结果应用Cosmed过滤器后,FVC、FEV1、PEF的测定值均比不用过滤器时有所下降,其平均差异分别为(-75.42±134.20)ml(P=0.011)、(-66.67±99.11)ml(P=0.003)和(-260.42±319.76)ml/s(P=0.001),其余指标均无明显变化。应用Microgard过滤器后,FVC、PEF、FEF200-1200的测定值较不用过滤器时下降,其平均差异分别为(-72.31±157.84)ml(P=0.028)、(-425.38±488.81)ml/s(P=0.0001)和(-193.08±371.5)ml/s(P=0.014),其余指标均无明显变化。应用Cosmed和Microgard过滤器对肺通气功能指标无显著影响。结论Cosmed和Microgard呼吸过滤器均引起呼气流速与容量的减小,然而这种差异很小,在通气功能指标的个体重复性变化范围内,并不会影响临床肺功能的诊断与评级。

OBJECTIVE To explore the factors and effective preventive method of nosocomial lower respiratory(infection) after general anesthesia through trachea intubation.METHODS Using bacterial culture and identification technique to detect the samples collected from pipe and the sides of anesthetic machine,from disposable virus/(bacteri)a respiratory filter and buccal and bronchial secretion of patients with general anesthesia through trachea(intubation.) RESULTS Eight of 15 anesthetic machines without disinfection were with positive bacterial culture.All sides of virus/bacteria respiratory filter,and buccal and bronchial secretion of patients during operation had same(bacteria.) No bacteria growth was found in anesthetic machine and disposable virus/bacteria respiratory filter used after operation.The same bacteria as from buccal secretion were isolated from anesthetic machine in patients(without) using virus/bacteris respiratory filter.CONCLUSIONS The(results) showed the anesthetic machine is more easy to be contaminated by patients,and the reuse of anesthetic machine is an important factor for nosocomial(lower) respiratory infection after general anesthesia through trachea intubation.Intensifying periodic disinfection of(anesthetic) machine and using virus/bacteria respiratory filter can prevent contamination between anesthetic(machine) and patients and decrease nosocomial lower respiratory(infection) after operation.

目的探讨气管插管全身麻醉患者院内下呼吸道感染的相关因素和有效的预防措施。方法从麻醉机回路内、病毒/细菌过滤器和气管插管全身麻醉患者口腔和支气管内吸取的分泌物做细菌培养。结果术前采样的15台未消毒的麻醉机中有8台细菌培养呈阳性;每例患者术中口腔、支气管分泌物及术后病毒/细菌过滤器均分离出相同细菌;使用一次性病毒/细菌过滤器的病例,术后从麻醉机内和一次性病毒/细菌过滤器麻醉机采样未捡出细菌;未使用一次性病毒/细菌过滤器的病例,术后麻醉机内可分离出与口腔相同的细菌。结论研究结果提示麻醉机很易受到手术患者的污染,而麻醉机的循环使用可将病原菌传播到下一个手术患者,是导致气管插管全身麻醉患者手术后下呼吸道医院感染的重要因素。
