The diagnosis and treatment of acute vascular injuries of extremities are not difficult, but still there are some important problems to be resolved, by which the curative effect can be improved. Seventy-three cases with acute vascular injuries of the extremities treated in our division have been studied retrospectively in a period of fourteen years from January of 1977 to May of 1991. We pointed out that the possibility of vascular injuries can not be ruled out by the finding of palpable distal pulses, higher incidence of nerve injuries usually associated with vascular injuries in the upper limbs, it is unnecessary to have angiography, thorough debridement should be carried out in wounded blood vessel and its surronunding soft tissues. The methods for vascular repairment should be properly selected; the treatment of the combined injuries could not be left out, and the nerve injuries should be repairea as soon as possible.

诊治四肢急性血管的损伤,如何才能收到良好的效果,目前仍然是一重要的课题。作者从1977年1月~1991年5月的73例中,发现了一些有关重要问题。指出在诊断时动脉搏动的存在不能排除血管损伤,尤其上肢血管损伤伴有较高的神经损伤发生率,血管造影并无必要;术中应对受伤的血管及软组织进行彻底的清创,选择恰当的血管修复方法,不要遗漏对复合伤的处理,注意神经损伤应早期修复。

目的探讨急性血管损伤的治疗方法。方法回顾 132例血管损伤患者的临床资料 ,并对其进行统计分析。结果本组病例血管损伤引起的死亡率为 6 8% ,截肢率为 8 3% ;手术后的通畅率达 91 9%。结论手术探查是诊治急性血管损伤的主要手段 ;采取灵活多样的方式处理血管损伤是确保通畅 ,降低病死率 ,保存肢体的关键

The Percutaneous Transluminal Coronary Angioplasty(PTCA) assisted with stenting has become one of the effective means to cure coronary heart disease.In practice,the match configuration of balloon and stent and the stent design altogether affect the stent′s instantaneous expanding results.The transitory non-uniform expansion of balloon-stent system, frequently named dogboning,is one of the main reasons to induce the acute vascular injury at stents both edges. This kind of vascular injury has a close relationship with the occurence of in-stent restenosis.In order to reduce the injury to decrease the in-stent restenosis,the transitorily expanding process of six different balloon-stent models were compared and analyzed by finite element method(FEM).The results suggested that the system's dogboning in expanding process could be effectively controlled by reducing the overlength of balloon and simultaneously increasing the strut width at the both ends of the stents.The results of experiments and simulation were consistent well,that indicated that FEM was one of the effective tools of design optimization of balloon-stent systems.

经皮穿刺冠脉支架技术已成为治疗冠心病的有效手段,在实践该技术过程中,球囊与支架的配套情况和支架的设计影响着支架瞬时膨胀的效果,球囊/支架系统在病变处的瞬时非均匀膨胀,即Dogboning现象,是造成支架端部急性动脉损伤的主要原因之一。这种损伤与支架内再狭窄(in stentrestenosis)有着密切的联系。为减少这种损伤,降低支架植入再狭窄率。本研究利用有限元技术对六组不同搭配的球囊/支架系统的瞬时膨胀情况进行了对比分析。结果表明,在球囊、支架的搭配中,减小球囊有效长度的过盈量和增加支架端部支撑体的筋宽,这两种方法共同作用,可以有效抑制球囊 支架系统在瞬时膨胀过程中的Dogboning现象。验证实验与模拟结果比较吻合,说明有限元法可以胜任球囊/支架的优化设计工作。
