ObjectiveTo investigate the effect of injecting botulinum toxin A(BTA) in patients with congenital nystagmus(CN).Methods22 CN patients were injected BTA,which was produced by Lan Zhou Biology Institute,into extraocular muscles directly.The changes of ocular nystagmus were measured by electronystagmogram (ENG) before injection and after injection of one week,one month,three months,and half a year.Statistic analysis was made in each group.ResultsThere was a significant difference between one week after injection group and pre injection group in amplitude on the primary eye position of close quarters ( P<0.05 ).There was significant difference between one month after injection group and pre injection group in all of the nystagmus data,except the frequency on the primary position of eyes in far distance ( P<0.05 ).In the three month and half a year after injection,the amplitudes of nystagmus in near and far distance primary position of eyes had significant differences comparing with the pre injection group ( P<0.05 ).The lowest intensity position although changed with no significant difference after injection of BTA,it can be seen that the position has the tendency of nearing the primary eye position by the analytic map of frequency table.ConclusionThis treatment on CN is effective in reducing nystagmus in the primary eye position.The most effective period was on one month after injection.On half a year,the effect reduced comparing with that of one month.The compensatory head postures are corrected in some degree.BTA injection has a good effect on the treatment of patients with CN.It is a new method for the clinical treatment of CN.

目的 观察肉毒杆菌毒素A(BTA) 治疗先天性眼球震颤(CN) 的效果,并进一步探讨其应用规律。 方法 对22 例先天眼震患者,用兰州生物制品所生产的BTA,以眼震图(ENG) 检查注射前与注射后1 周,1 ,3 ,6 月的近、远距离眼震情况,各组进行统计学处理,并比较统计学差异。 结果 注药后1 周与注药前相比近距离原眼位眼震振幅降低有明显统计学差异( P< 0 .05) ;注药后1 月与注药前相比,除近距离原眼位震频降低无明显统计学差异( P> 0 .05) 外,其它远、近距离原眼位的各眼震参数降低均有明显统计学差异( P< 0 .05) ;注药后3 ,6 月与注药前相比近、远距离原眼位振幅降低有明显统计学差异( P< 0 .05) ;最小震强位置注药后1 周,1 ,3 ,6 月与注药前相比虽无显著差异,但从频数分布图中可见最小震强出现的位置有向原眼位集中的趋势。 结论 该方法治疗先天性眼球震颤可使近、远距离原眼位眼震有所减轻,代偿头位有所改善。注药后1 月疗效最显著,注药后3 月疗效开始回退,但可重复注药。该方法治疗先天性眼球震颤有明显效果,可成为今后临床治疗先天性眼球震颤的新方法

ObjectiveTo investigate the morphologic changes of the effect of botulinum toxin A(BTA) into extraocular muscles.MethodsHistochemical methods including hematoxylin and eosin stain and gold chloride stain were used to study the extraocular muscles of rabbits at different time period after BTA injection by light microscope.Ultrastructural changes were determined by electron microscopy.ResultsOne week after BTA injection,the amount of sarcoplasmic reticulum had expanded,and the basement color of the mitochondria and the amount of muscle glycogen increased.One month after injection,segmental myofibril spasm was seen.Two months after injection,the extraocular muscle became atrophic.ConclusionAt the site of the BTA injection in the muscle irreversible atrophy was found in segments, where the nerve terminals regenerate after injection,the myofibrils demonstrate focal hyperplasia in suggesting the muscle function will partly recover.

目的 进一步观察肉毒杆菌毒素A(BTA)所致的眼外肌形态学改变并探讨其作用机制。方法 用组化 3种方法 (HE染色 ,BEST卡红法 ,1%氯化金染色 )对注射BTA后不同时间的兔眼外肌行光镜下组织结构检查 ,并用透射电子显微镜观察其超微结构的变化。结果 注射BTA后 1周出现肌浆网的增多和扩张 ,线粒体的基质着色加深 ,肌糖原颗粒增多。注药后 1月眼外肌出现肌糖原纤维的节段性痉挛。注药后两月眼外肌出现不可逆萎缩。结论 BTA作用部位的眼外肌纤维出现节段性不可逆萎缩 ,随注药后时间的延长神经肌肉接头处神经再生 ,从而产生肌纤维的节段性增生 ,使眼外肌力得到部分或全部的恢复。
