Al particles were implanted into Ag substrate by plasma based ion implantation with Al plasma (AIPBII). The depth profile and chemical state of Al in implantation layer were acquired by XPS. The phase composition of Al implantation layer and that of Al deposition layer by unbalance magnetron sputtering (UBMS) were determined and compared. The glanced X-ray distraction method (GXRD) was used to analysis the phase in the implantation layer in detail. The results showed the Al concentration along depth direction decreases gradually. Ag atoms can be found in the surface layer of implantation sample. The Ag-Al solid solution and a few μ-Ag3Al phase exist in this surface layer. In the deep layer of implanted sample aluminum oxide was found and coexists with Ag-Al solid solution and μ-Ag3Al. The Ag-Al solid solution disappears at the deeper layer when the Al concentration decreased rapidly, and μ-Ag3Al coexists with aluminum oxide. Ag3AI phase has β-Mn complex cubic structure and the Al concentration in Ag3AI phase fluctuates in a large range. No Ag3AI phase was found in the deposition layer of Al.

Objective To compare the effect to cervical curve and intervertebral height of different methods of intervertebral distraction.Methods 54 cases of cervical spondylosis treated with anterior decompression and fusion between July 1995 to March 2001 were included and divided into manipulation distraction group, Cage distraction group and Caspar distractor group, depending on different methods of distraction during operation. The D value of cervical curve and intervertebral height before and after operation were measured respectively and compared statistically.Results D value and intervertebral height in Cage distraction group and Caspar distracter group is significantly greater than those in manipulation distraction group (P<0.01).Conclusion The degree of restoration of cervical curve and intervertebral height can be affected mainly by the distraction methods used in the operation, distraction by Caspar distractor is an ideal method for restoration of cervical curve and intervertebral height.

目的 分析颈前路减压术中不同撑开技术对恢复颈椎生理曲度及椎间高度的影响。方法  1995 .7— 2 0 0 1.3月间施术的颈椎病患者 5 4例 ,按照术中椎间隙牵开方法的不同分为徒手牵开组、Cage牵开组及牵开器牵开组等三种。分别测量各组术前、术后颈椎生理曲度及椎节高度值 ,测量结果进行统计学比较。结果 牵开器牵开法及Cage牵开法较徒手牵开法颈椎生理曲度及椎节高度增加值大 ,且具显著性差异 (P <0 .0 1)。结论 术中牵开技术可影响颈椎生理曲度及椎节高度的恢复 ,牵开器牵开法对改善颈椎生理曲度及椎节高度较为理想。

[Objective]To study the safety,clinical effect and therapeutic characteristics of distraction apparatus for correcting flexion contracture deformities of the knee joint by evaluating the clinical outcomes and investigating the results of biomechanic test.[Method]Forty-nine patients (included 52 knees) were treated by the standardized distraction apparatus and the comprehensive assessment standard was adopted to evaluate the clinical outcomes.Five patients were examined by three-dimensional CT of the knee joints pre-operatively and post-operatively to observe the changes in the knee structure.To observe the effect of distraction on the popliteal artery and the limb nerves,the popliteal artery diameter and blood flow speed were examined in 15 patients and the regio cruris electromyogram and nerve conduction velocity were examined in 6 patients before and after treatment.Three different means of distraction and measurement were underwent in 3 patients so as to observe the changes in the flexion curve and force of posterior soft tissues of the knee.[Result]All patients (52 knees) regained congruous structure and better function,the final excellent and good result rate was 65%.Three-dimensional CT examination of 5 patients showed that the postoperative knee joint structures were superior to the preoperative ones and there weren't damage of articular surface.The statistic results indicated that the popliteal arteries weren't obviously disturbed by distraction,whereas the limb nerves were probably disturbed temporarily.Three methods of distraction and measurement in 3 patients showed different outcomes,indicating that the effect of distraction on soft tissues was influenced by the distraction method,the ages of patients and the extents of deformities of the limb.[Conclusion]The distraction speed of therapeutic method is safe,reasonable and effective in correcting flexion contracture of the knee joint;The distraction force on posterior soft tissues of the knee is influenced by various elements,thus,the distraction speed should be individualized.Low-load and steady stretch are advantageous to soft tissue regeneration and safer.

[目的]通过对膝关节牵伸器矫治膝关节屈曲挛缩畸形及生物力学测评结果研究,论证该技术的安全性、临床疗效及治疗特点。[方法]49例患者(52个膝),应用标准的牵伸器治疗,采用综合评定标准评价临床结果;5例治疗前后膝部三维CT重建,观察关节结构的改变;检查15例治疗前后腘动脉的直径、血液流速及6例小腿肌电图、神经传导,以观察牵伸对腘动脉及肢体神经的影响;对3例患者分别以3种不同的牵伸及测试方法,初步观测不同的牵伸过程中屈曲角度与膝后软组织受力变化。[结果]52个膝关节功能均有恢复,最终优良率为65%;5例CT三维重建显示术后膝关节结构均较术前合理,关节面无破坏。统计结果显示牵伸过程对腘动脉没有明显的影响,可能对小腿神经有短暂的影响;3例患者用3种不同的牵伸及测试方法,最后结果均不同,说明软组织受力情况与牵伸方法、患者年龄及屈膝畸形程度有关。[结论]该治疗方法是安全的、合理的、有效的;膝后软组织受力受到多种因素的影响,因此牵伸速度应个体化,低负荷均匀的受力可能更有利于软组织的再生、相对更安全。
