Of 39 patients with IDDM who had been grafted with 1-2-wk cultured islet tissure of human fetal pancreas, 38 cases were proved to be successful and 1 ineffective. Following transplantation, the average daily insulin requirements and average fasting plasma glucose concentrations of the 38 recipients dropped from 46.97±13.63 u and 207.03±72.92 mg/dl to 18.16±9.08 u, (p<0.001) and 161.07±57.03 mg/dl, (p<0.01) respectively. Two other diabetics had ever been non-insulin dependent over a period of time, too. In 9 cases the average fasting seurm G-peptide levels rose from 0.046±0.017 pmol/ml preoperatively to 0.108±0.055 pmol/ml post-operatively (p<0.01). The latest average fasting serum C-peptide levels of 25 other were 0.182 ± 0.071 pmol/ml, though the assays were undone prior to transplantation. 38 recipients have been followed up for 18.93±8.42 mo, and the islet allografts have been functioning well without any rejections.

本文报道39例胰岛素依赖型糖尿病患者经胰岛移植后已随访6月以上(6.5~32月,平均18.93±8.42月)临床研究资料。应用经短期培养的人胎胰胰岛作腹膜腔内或腹直肌内移植后1例无效,38例有效。有效者每日胰岛素用量于移植后比移植前平均减少61.34%,血糖控制显著改善,3例已分别完全停用胰岛素治疗9月、6月与1月许,空腹血清C肽浓度正常,病情控制良好;另2例曾短期停用胰岛素。38例有效患者于移植后未常规应用免疫抑制治疗而移植物功能均仍保持良好,无1例发生排斥反应。

No exicrube function was detected as evidenced by the low amylase levels.It is concluded that mioroenoapsulated human fetal pancreatic tissue retains viability for more than 25 days and may be expectantly useful as transplants in the treatment of diabetes mellitus.

作者对50例主要取自水囊引产的4至6月龄人胎的胰腺,在机械分散、体外培养4至7d后,用海藻酸钠、多聚赖氨酸等高分子生物材料进行微囊化,并在体外培养条件下采用免疫酶标组织化学染色(PAP法)、放射免疫测定法、微量氧化酶法,研究微囊化人胎胰组织的存活、分化、增殖情形及激素和外分泌酶分泌功能。结果表明:微囊化人胎胰组织体外培养48d,β细胞分化增殖良好。培养条件下25d仍保留有良好的胰岛素、C肽分泌功能,与未微囊对照组织无明显差别。未见明显的淀粉酶分泌功能。实验提示微囊化人胎胰组织可望成为一种有前途的移植物,供临床治疗胰岛素依赖性糖尿病。

采用 Hanks’(4℃)液自胰管灌注使胰腺膨胀,或用多点注射法使胰腺浆膜扩张,整个胰腺则清晰可见,此时迅速摘除并清除它周围的血管及结缔组织,用Hanks’液清洗整个胰腺并将它快速剪碎,然后加入胶原酶、胰酶消化,再经过滤和 Ficoll 密度梯度离心,使人胎胰腺中的胰岛有效地被分离并将其收集。胰岛的纯度达80%,通过体外活染,表明胰岛具有活性。通过放免法测定,游离胰岛及胰岛分散为单个细胞的分泌能力正常,在离体中对葡萄糖急性刺激能产生反应。
