METHODS: Periapical biofilm samples of 25 teeth with periapical periodontitis were collected by periapical surgery and bacterial samples of root canals were taken from 20 of 25 teeth. All the samples were inoculated, separated and purified according to usual practice. Identification of pure cultures was based on aerotoleranee, colony and cell morphology, colony pigmentation, Gram-staining of cells and biochemical tests. The rates of detection and constitution of bacteria in periapical biofilm were statistically analyzed and compared with that in root canals. RESULTS: Microbial study showed that 23 of 25 periapieal biofilm specimens yielded microbial growth (92.0%) and 27 different microbial species were detected. 2~4 bacterial species in the same plate were detected from 80.0% of the specimens, the rate of G+ species detection was 80.0%, the ratio of constitution was 54.6%; the rate of G- species detection was 84.0%, the ratio of constitution was 45.4%; the rate of facultative anaerobes detection was 44.0%, the ratio of constitution was 39.6%; the rate of obligate anaerobes detection was 80.0%, the ratio of constitution was 60.4%. The most prevalent bacteria were Stomatococcus, black-pigmented Gram-negative short bacilli, black -pigmented G - short bacilli, Actinomyces, Peptostreptococcus, Fusobacterium, Hemophilus, Propionibacterium acnes. Veillonella parvula, Capnocytophaga in turn. Bacterial detections of infected root canals were almost the same as that of periapical biofilm. Bacterial study also showed that there was no obvious difference regardless periapical lesions were different and whether the teeth had received root canal therapy. CONCLUSION: (1) 92.0% of periapical biofilm specimens yielded bacterial growth, several bacterial species existed in 80% specimens; (2) The species、rates of detection and constitution of which showed that bacteria of periapical biofilm came from infected root canals; (3) There was no obvious difference in bacterial bacteria in periapical biofilm were almost the same as that in root canals, which showed that bacteria of periapical biofilm came from infected root canals; (4) There was no obvious difference in bacterial detection of periapical biofilm in spite of different periapical lesions.
