The control gro up (35 cases) was given routine treatment, but the treatment group (38 cases) ad ded with thermal therapy, computerized medium frequency electrotherapy, manual t herapy, etc. After two courses, therapeutic effects were observed and assessed w ith standards for hip joint function.ResultsOf th e control group, 14 cases were cured, 9 effectual, 9 effective, and 3 ineffecti ve; of the experiment group, 23 cured, 9 effectual, 5 effective, and 1 ineffecti ve. By statistical analysis, the therapeutic effect in the treatment group is su perior to those in the control group ( P<0.05). Hip joint fu nction is assessed by scores as: the median point before and after the treatment for the control group is 18.00, the experiment group is 27.50, with statistical ly significance difference ( P<0.01).The average cure time f or the experiment group is 11.04±5.10 days, fewer than that for the control gro up (14.79± 5.79 days)(P<0.05).Conclusions Comb ined treatment used for piriformis syndrome can produce remarkable therapeutic e ffect, and various types of therapies can play such coordinate roles as promotin g blood circulation, repercussion, anti-inflammation, analgesia, relaxing adhes ion and softening scar.

目的观察综合疗法治疗梨状肌综合征的康复效果 ,并分析梨状肌综合征的病因、病机、病理及康复机理。方法将73例梨状肌综合征患者随机分为对照组 (35例 ,予常规治疗 )和治疗组 (38例 ,增加温热疗法、电脑中频疗法、手法治疗等 ) ,两个疗程后观察效果 ,并用李氏髋关节功能评定标准评定疗效。结果对照组痊愈 1 4例、显效 9例、有效 9例、无效 3例 ;治疗组痊愈 2 3例、显效 9例、有效 5例 ,无效 1例 ,经秩和检验 ,治疗组疗效优于对照组 (u =2 .0 0 ,P <0 .0 5)。治疗前后髋关节功能评分差值中位数为 ,对照组 1 8.0 0分 ,治疗组 2 7.50分 ,两组比较有高度显著性差异 (u =2 .98,P <0 .0 1 )。治疗组平均治愈天数为 (1 1 .0 4±5 .1 0 )天 ,短于对照组 (1 4 .79± 5 .79天 ) (t =2 .0 6 ,P <0 .0 5)。结论综合治疗梨状肌综合征的康复效果显著 ,可充分发挥各种疗法的协同作用 ,具有促进血液循环、消炎消肿、止痛解痉、软化疤痕、松解粘连等功效。

BACKGROUND: Hypertrophic scar results from imbalance between local collagen synthesis and degradation and abnormal aggregation of collagen after burn and trauma. OBJECTIVE: To observe the effects of collagenase on collagen degradation in nude mice with graft of hypertrophic scar and its therapeutic effects on hyertrophic scar.DESIGN: Controlled experiment was designed.SETTING: Department of Rehabilitation and Physical Therapy,Southwest Hospital, Third Military Medical University of Chinese PLAMATERIALS: The experiment was performed in Experimental Center of the Third Military Medical University of Chinese PLA from July 1995 to April 1997, in which, 10 specimens of hypertrophic scar were employed, collected from the removed hypertrophic scars in Department of Plastic Surgery. All of patients were in the known before the operation. Experimental animals were 15 BACB/C nude mice, of which, 8 mice were male and 7 mice female. Totally 15 mice were grafted with hypertrophic scar tissue, 10 mice of which were survived after once graft; with second graft in the rest mice, 2 mice were survived and 3 mice were dead. The survival nude mice were randomized into experimental group and the control, 6 mice in each one.METHODS: The removed hypertrophic scars in plastic operation were grafted in the wound on the dorsum of nude mice to establish graft model of hypertrophic scar. In experimental group, 1% collagenase was injected locally in scar tissue; in the control group, collagease solvent was injected locally, once a week, for 4 weeks totally. After treatment, the materials were collected for macroscopic observation and observation and analysis with optic and electronic microscopes. MAIN OUTCOME MEASURES: ① Macroscopic observation of scar tissue before and after treatment. ② Optic microscopic observation of scar tissue before and after treatment. ③ Electronic microscopic observation of scar tissue before and after treatment.RESULTS: Twelve nude mice were survived after scar graft and all of them entered result analysis, 6 mice of which were in experimental group and 6 mice in the control group. ① In the experimental group, the scar was smaller in size, thinner in thickness and softer in quality compared with the control group, indicating quite significant difference. ② With HE staining, Van Gieson's collagenous fiber staining and compound staining on slices, it was displayed that in experimental group, dermal layer became thin, collagenous fiber was unclear in structure and disperse in arrangement, but in the control, the dermal layer of scar tissue was hypertrophic with rich collagenous fibers and disarranged structure. ③ With electronic microscopic observation, in experimental group, collagenous fibers were destroyed and unclear in structure, but in the control, collagenous fibers were disturbed in arrangement with distinct strips and clear structure. CONCLUSION: Collagenase degrades collagen of hypertrophic scar, lessens and softens scar, suggesting that local injection of collagease is probably a good therapy for hypertrophic scar.

背景:肥厚性瘢痕是是烧伤和创伤后局部胶原合成与降解平衡失调,胶原异常聚集的结果。目的:观察胶原酶影响裸鼠移植肥厚性瘢痕胶原降解的过程,以及其对肥厚性瘢痕的治疗作用。设计:对照实验。单位:解放军第三军医大学西南医院康复理疗科。对象:实验于1995-07/1997-04在第三军医大学实验动物中心完成。肥厚性瘢痕标本10例,均为第三军医大学西南医院整形外科手术切除的肥厚性瘢痕,术前已征得患者同意。实验动物:BAVB/C裸鼠15只,雄性8只,雌性7只。共移植肥厚性瘢痕组织15只,一次存活10只,余2次移植,存活动物2只,死亡3只。将存活裸鼠随机分为胶原酶治疗组和对照组,每组6只。方法:将整形手术切除的肥厚性瘢痕移植于裸鼠背部创面,建立肥厚性瘢痕裸鼠移植模型。胶原酶治疗组局部注射1%胶原酶于瘢痕组织,对照组局部注射胶原酶溶解液,1次/周,共4周,治疗结束后取材行肉眼观察、光、电镜观察和分析。主要观察指标:①治疗前后瘢痕组织的肉眼观察结果。②治疗前后瘢痕组织的光镜观察结果。③治疗前后瘢痕组织的电镜观察结果。结果:移植瘢痕后实验裸鼠存活12只,均进入结果分析,胶原酶治疗组6只,对照组6只。①胶原酶治疗组瘢痕面积变小、高度变...

Objective:To study the earlier prevention and treatment of scars in facial surgical trauma by using intense pulsed light in conjunction with gel silica membrane.Methods:We used the intense pulsed light photon therapy equipment named IPL QUEE,made in Wuhan Qizhi company,with the density of output energy as 20~23J/cm2,the area of light spot as 8mm×34mm, the treatment parameter as 560nm and pulse width as 2~6ms.All cases were treated by intense pulsed light one time per 4 weeks,with a course of treatment of 4~6 times.The gel silica membrane named Cica-Care was pasted on scarring part and each membrane staying more than 8 hours per day,which continued 3~6 months.Results:Intense pulsed light with characteristics of penetrating skin,being selectively absorbed by paraplasm micrangium in injured part and being changed into heat energy,could allow blood vessel quickly to close and decrease the denaturation of fibroblast,and meanwhile could cause skin pigmentation to split.Therefore,intense pulsed light could make scars even and soft and dispel red spot and pigment due to the absorption and metabolisim of highlight by autologous tissue,and at the same time the gel silica membrane could soften scar tissue,for the silicone rubber membrane had the functions to reduce evaporation of moisture content in scars and water-soluble substances in the interstitial tissue and to descend the pressure of fluid mechanics.In all the 27 cases,15 cases were completely healed,and 12 cases were improved.Conclusion:Earlier Preventing and treating scars in facial surgical trauma by using intense pulsed light in conjunction with gel silica membrane is a better therapy approach.

目的:应用强脉冲光联合硅胶膜片行颜面外伤早期预防和抗瘢痕治疗。方法:应用武汉奇致公司生产的IPL QUEE强脉冲光子治疗仪,输出能量密度为20~23J/cm2,光斑面积8mm×34mm,治疗参数560nm脉宽:2~6m s。每4周一次,进行强脉冲光治疗,4~6次为一疗程。硅胶膜片为:瘢痕敌胶片。于瘢痕形成部位外贴,每天8h以上,连续应用3~6月。结果:利用强脉冲光能穿透皮肤,被创伤部位异常增生的毛细血管选择性吸收并转化为热能的特性,使血管快速封闭,成纤维细胞变性减少,同时强光可使创伤后皮肤沉着的色素裂解,从而被自体组织吸收、代谢;由此使瘢痕平软,红斑、色素减退。同时,硅胶膜使瘢痕水分蒸发减少,皮肤内水分转移到角质层,间质水溶性物质减少,流体力学压力下降,瘢痕组织因而软化。27例患者中,15例痊愈,12例有效。结论:认为强脉冲光联合硅胶膜片行颜面外伤早期预防和抗瘢痕治疗是一种疗效好的方法。
