After the treatment with antibiotics the results of the test all returned to normal, indicating that there were bacteria overgrowth in small intestine in these patients and the incidence was 13.05%. The mechanism, clinical manifestations and treatment of bacteria overgrowth in small intestine caused by cimetidine in peptic ulcer patients were discussed in brief.

采用CM型氢气微分析仪,对46例溃疡病人在服用甲氰咪呱过程中进行葡萄糖呼吸试验,发现6例肺泡呼出气含氢量>20ppm,即呼吸试验阳性。服用抗生素后复查葡萄糖呼吸试验均转阴性,提示6例阳性病人存在小肠细菌过度生长,发生率为13.05%。扼要介绍葡萄糖呼吸试验的原理、溃疡病人服用甲氰咪呱后引起小肠细菌过度生长的机理、临床表现及治疗方法。

Hydrogen level in the exhaling air after oral lactose or glucose loading was determined in normal subjects and patients after gastreetoaiy. Basal hydrogen was less than 30 ppm in all the 16 normal subjects. After lactose ingestion, 13 subjects out of the 16 (81.2%) showed an increass of hydrogen excretion of 20 ppm or more. In 7 patients with Billroth I gastrec- tomy and 14 patients with Billroth II gastrectomy, increase of hydrogen excretion after lactose ingestion occurred in 6 out of the 7 (85.7%) and 12 out of the 14 (85.7%) respectively, which is not significantly different from that of the control. After glucose intake, increase of hydrogen excretion occurred in 1 out of the 7 (14.2%) and 5 out of the 14 (35.7%). The elevation of hydrogen in the exhaling air indicates the overgrowth of bowel bacteria. Our findings suggest that hydrogen breathing test after lactose loading is insensitive to reveal this phenomenon in patients after gastrectomy, but after glucose loading, the test might carry out this function especilly in those with Billroth II operation.

本文采用口服乳糖及葡萄糖后氢呼吸试验的方法对正常人及胃切除术后患者进行研究。结果正常人乳精呼吸试验阳性率81.2%,葡萄糖呼吸试验均正常。7例毕Ⅰ式及14例毕Ⅱ式术后患者乳精试验阳性率与正常人无显著差别(均为85.7%),但葡萄糖呼吸试验阳性率升高(14.2%~35.7%)。提示胃切除术后对乳糖呼吸试验无影响,葡萄糖呼吸试验异常,其中毕Ⅱ式较毕Ⅰ式明显。

OBJECTIVE: To find out the tolerance to lactose of Chinese. METHODS: Hydrogen breathing test (HBT) was used in 136 healthy adults of the Han nationality with different quantity of lactose A, B, C. The different groups of subjects were given 25 g, 12.5 g, 6.25 g lactose respectively. The subjects of the C group taking lactose with HBT negative were given 12.5 g lactose again were included the in D group. There was no significant difference between the B and D groups (P>0.05). CONCLUSION: The tolerance quantity of lactose for Chinese is 6.25 g. Taking 125 ml milk for each time is reasonable.

目的:探讨国人对乳糖耐受性。方法:采用不同剂量乳糖进行氢呼气试验。对136名健康汉族成人分为3组,A,B,C组乳糖负荷量分别为25g,12.5g,6.25g。D组为从C组氢呼气试验阴性中再给予12.5g乳糖重复试验。结果:A,B,C,D组乳糖吸收不良发生率分别为85.0%,48.9%,2.0%和46.7%;乳糖不耐受率分别为54.3%,30.4%,2.0%和20.0%。A,B,C组乳糖吸收不良发生率及乳糖不耐受率均有极显著性差异(P<0.001);B,D组间均无显著性差异(P>0.05)。结论:国人乳糖耐受量为6.25g,饮用牛奶每次以125ml为合理营养。
