Methods Six patients underwent aortic reconstructions or partial aneurysm resections and one did hepatic cavernous hemangioma resection in general anesthesia. Cell saver was used for perioperative blood salvage and the washed blood was reinfused to all patients respectively during operation. Results The average total blood loss was 3 107ml(1 300~ 8 500ml), and the cell saver blood salvaged per case was 1 125ml(675~2 925ml).Seven patients were infused with an average of 2 000ml gelofusine or haemaccel. Two cases were infused with bank blood and one with fresh frozen plasma. Conclusion The uses of perioperative autologous blood salvage and cell saver techniques may decrease the need for homologous bank blood transfusion in complex great vessels surgery and could reduce the rate of infective complications.

目的 介绍全自动血液回收系统在复杂大血管手术中应用的经验。 方法 6 例动脉瘤和1 例肝血管瘤手术患者,在全身麻醉下行人造血管置换或动脉瘤部分切除术,在手术开始或体外循环结束时,启用全自动血液回收系统,回收术野的出血或渗血,进行浓缩、洗涤后,再回输入体内,并按回收浓缩洗涤回输程序重复进行。 结果 7 例患者平均回收血量3 107ml(1 3008 500 ml) ,洗涤红细胞1 125ml(6752 925ml) 。平均输注佳乐施或海脉素2 000ml,2 例术中输入全血和血浆,术中血流动力学稳定。术后如期出院。 结论 复杂的大血管手术应用全自动血液回收系统具有广泛的适应证,是节约用血、减少输用库血的可靠方法。

Objective:To investigate the application of the device and technique of extracorporeal circulation to autotransfusion in large vessel surgery.Methods:We collected the lost autogenous blood by device extracorporeal circulation and then transfused it back to the patient intraoperatively according to the mean arterial pressure(MAP).The lost blood volume,the collected blood volume,the autotransfused blood volume,the free hemoglobin content of the patient 24 hours after operation and the volume and colour of the urine were all recorded and the blood was cultured as well.Results:The average volume of blood loss was(4 916±188.5)ml and the average collected blood volume was(4 530±135.7)ml.The recollection rate was 92.1%.The output volume of urine exceeded 50 ml/h intraoperatively and postoperatively.The colour of the urine was bright without hemoglobin.The collected blood and the patients blood 24 hours after operation were cultured and no bacteria were found.Conclusions:The application of extracorporeal circulation equipment and technique to autotransfusion in the large vessel surgery can save blood,reduce transfusion morbidity and maintain the blood volume.

Objective To study the role of mild hypothermic circulatory arrest (MHCA) technology in complicated head and neck big vessel surgery.Methods Two-h pMCAO rats,which were substituted for head and neck surgery disease animals,underwent cooling to 26 ℃ under MHCA technique by CCFFB methods.In control group 1 (n= 12), pMCAO rats were treated under normal temperature,while those in group 2 (n= 16) were subjected to 45-min of MHCA at 26 ℃ with cooling for 30 min and remarming 30 min after MCAO.Then the neural morphological region of the hippocampi was stained by HE,and the expression of apoptosis gene bcl-2 and bax was detected.Results Little morphological change was observed in MHCA group.The expression of antiapoptosis gene bcl-2 in MHCA group was significantly increased as compared with other groups,whereas the expression of proapoptosis gene bax had no change.The cerebral infarct and core infarct volums in MHCA group were reduced by 38.36% and 46.5% as compared with group 1 respectively (P< 0.05). Conclusion Complicated head and neck vessles surgery can be done safely and effectivly under the aid of MHCA technology.
