Objectivet To explore the changes of biomechanical properties of cancellous aIlograft and its relationship with different biomechanical environment. Methodf Cancellous allograft was transplanted to each side of the midshaft diaphyseal ulnar of 40 rabbits. The left transplanted allograft underwent normal physiological load, while the right underwent lower load. Animals were kiIled and specimens taken for examination of bone mineral density, three-point-bending test and mean trabecular thickness. Results: Bone mineral density, maximal bending moment and mean trabecular thickness of graft were found to have been increasing constantly. At the sixteen week after transplantation, the above data of normal loaded graft is better than that of lower loaded graft (P<0.01,0.05,0.05 respectively). Conclusionf Changes of biomechanical properties of cancellous allograft shows more apparent correlation with the magnitude of load to which the graft would be subject.

目的:探讨同种异体松质骨移植后的力学性能变化以及不同力学环境对其的影响。方法:在40只家兔前肢尺骨中段进行同种异体松质骨移植,并使左右侧植骨块分别承受正常生理载荷与低载荷,动物分批处死后取出植骨区标本进行骨密度值、三点弯曲、平均骨小梁厚度等测试。结果:同种异体松质骨愈合时的骨密度值、最大弯矩、平均骨小梁厚度逐渐上升。在移植后第16周时,和低载荷侧比较,正常载荷侧的上述指标都明显优于低载荷侧(P值分别<0.01,0.05,0.05)。结论:同种异体松质骨移植后的力学性能变化与它所承受的载荷大小有较强的相关性。

Objective To explore the difference of incorporation pattern of conical allograft and cancellous allograft. Methods Freeze - dried allograft were transplanted to left side of the midshaft diaphyseal ulnar respectively in each group of 32 rabbits. Animals were killed in batch at two. four. eight and sixteen weeks after transplantation and specimens were taken for in situ hybridization of the type I collagen gene and the histomorphometric analysis of porosity of conical bone and mean widths of trabeculae. Results Many bone resorptive cavities occured early in the conical allograft followed by the appearance of osteoblast expressing type I mRNA. The bone porosity increased gradually and then decreased slowly. A lot of osteoblast expressing type I mRNA deposited on the surface of old trabeculae at the early stage and osteoclasts could be observed nearly simultaneously. The mean widths of trabeculae increased constantly. Conclusion The osteogenesis of conical allograft is based on the osteoclastic resorption while new bone could be formed directly in cancellous allograft.

目的探讨同种异体皮质骨和松质骨愈合方式的差异。方法兔左侧尺骨中段植人冻干异体骨,分为皮质骨组和松质骨组,均为32只。术后2、4、8、16周分批处死,取材进行Ⅰ型胶原基因的原位杂交研究,以及皮质骨孔隙率和松质骨平均小梁宽度的组织学定量分析。结果异体皮质骨内先出现大量的骨吸收陷窝,然后在陷窝内出现表达1型胶原mRNA的成骨细胞;其孔隙率先增大,然后缓慢减小。异体松质骨内早期就有大量表达五型胶原mRNA的成骨细胞沉积在原有骨小梁表面,附近同时有破骨细胞的活动;其平均小梁宽度持续上升。结论同种异体皮质骨的愈合是在吸收的基础上成骨,而同种异体松质骨内则可以直接成骨。

Objective To investigate the results of using bone allograft for repairing bone defects. Methods Twenty cases with bone defects were treated with bone allograft in last 5 years. They were followed up averaging 37 months after operation.The healing of the site of the bone allograft was observed. Conclusion The clinical result showed that the incorporation of cancellous allograft is better than that of cortical bone, best results could be achieved by intercalary implant with good bony apposition and firm internal fixation. Bone allograft is an effective method for treating bone defect in clinical practice.

目的 研究同种骨移植治疗骨缺损的疗效。方法  2 0例接受同种骨移植的骨缺损患者平均随访 37个月 ,观察术后植骨部位的骨愈合情况。结果 术后除 1例因感染而失败外 ,其余均达到骨性愈合或基本愈合。结论 同种松质骨移植后的骨愈合优于皮质骨 ,插入植骨、紧密对位、坚强内固定有利于植入骨的愈合。同种骨移植是临床治疗骨缺损的一个有效方法
