目的 :探讨脑疾病患者脑脊液 ( CSF)渗透压、血浆渗透压、p H值及电解质的变化。方法 :对脑疾病患者 CSF渗透压、血浆渗透压、p H值及电解质进行测定。结果 :脑出血急性期和细菌性脑膜炎患者的 CSF及血浆渗透压、CSF中 K+明显高于正常组 ( P <0 .0 1) ,p H显著低于正常组( P <0 .0 1)。脑梗死患者的 CSF及血浆 Ca2 +明显降低 ( P <0 .0 1)。结论 :监测 CSF、血浆渗透压、p H值及电解质对判断脑疾病患者的病情预后有一定价值。

Objective: To evaluate the cerebral hemodynamics and the effect of brain hyperemia in acute head - injured patients with intracranial hematomas (ICH) evacuated. Methods: The velocities of hasal cerebral arteries and extracranial internal carotid arteries were dynamically monitored by transcranial Doppler sonography in 24 acute head - injured patients after their ICH were removed. The intracranial pressure (ICP) were checked through cramal decompressive windows. Results: Two cases died due to brain hyperemia. High ICP occurred in 7 of 8 cases who had been undergone cranial decompression in 12 patients of hyperemic group. The rate of high ICP of hyperemic group was higher than (that) of the nonhyperemic group. Conclusion: Brain hyperemia takes place mostly in bilateral cerebral hemispheres and may result in extensive brain edema and high ICP. High ICP crisis can occur in the acute head -- injured patients with severe brain hyperemia.

探讨急性颅脑损伤后脑血流动力学改变以及脑充血对颅内压的影响。方法:选用急性颅脑损伤颅内血肿术后患者24例,用经颅多普勒超声仪动态监测脑底动脉及颅外颈内动脉的血流变化,通过颅外减压窗观察颅内压的变化。结果:脑充血组 12例中 8例同时行颅外减压术,在脑充血期间 7例发生颅内压增高,颅内压增高发生率高于无脑充血组(P< 0. 05)。 2例因脑充血死亡。结论:脑充血多为双侧性,可引起较广泛的脑水肿及颅内压增高,脑充血严重时可出现颅内压增高危象。

Objective To evaluate the clinical significance in the detection of uromicroprotein levels in acute head injured patients.Methods The patients with head injury were divided into 4 groups according to GCS(Glasgow coma scale) score: the most severe head injury group,severe head injury group,moderate head injury group and mild head injury group.The levels of urinary α 1 microglobulin(α 1 MG), microalbumin(MA) , transferring(TRF) and immunoglobulin G(IgG) were determined in all patients. Results The correlation analysis showed that the levels of uromicroprotein were negatively correlated with GCS score.Multiple stepwise regression was used to reveal that the levels of α 1 MG and IgG were closely related to the GCS score.Conclusion The determinations of uromicroprotein are useful for evaluating the severity of brain damage in patients with acute head injury and its kidney damage.

目的 评价脑外伤患者尿微量蛋白测定的临床意义。方法 根据GCS评分将急性脑外伤患者分为特重型组、重型组、中型组、轻型组 ,检测尿α1 微球蛋白 (α1 MG) ,微量白蛋白 (MA) ,转铁蛋白 (TRF)和免疫球蛋白G(IgG)。 结果 相关分析显示 ,α1 MG、MA、TRF和IgG与GCS均呈负相关。应用多元逐步回归分析法显示 ,α1 MG和IgG值与GCS计分值密切相关。结论 脑伤后早期尿α1 MG ,MA ,TRF和IgG检测可作为脑外伤程度诊断的辅助指标 ,对诊断脑外伤与肾损害的程度有一定参考价值。
