A Neurophysiological Picture of the Genesis of Fatigue, Chronic Fatigue, and Overfatigue in Human Operators
      
The present widespread conceptual model of the neurophysiological picture of human fatigue, chronic fatigue, and overfatigue is limited to the picture of hypoexcitability and development of inhibitory processes in the CNS.

Objective:To study the differential diagnoses of patients with medically unexplained chronic fatigue and its relationship with neurasthenia.Method: 96 outpatients with a history of medically unexplained fatigue for 3 or more months were assessed,using a structured clinical interview which generated DSM-Ⅲ-R diagnosis,ICD-10 and CCMD-2-R diagnosis for neurasthenia,and CDC diagnosis for chronic fatigue syndrome.Results:26 subjects received major axis I diagnosis of DSM-Ⅲ-R.Among the other 70 subjects who satisfied the CCMD-2-R criteria for neurasthenia,62 could be diagnosed with neurasthenia and 8 with undifferentiated somatoform disorders according to ICD-10.Using DSM-Ⅲ-R,58 of the 70 subjects were diagnosed with undifferentiated somatoform disorder,while 12 did not receive any diagnoses.Only 13 subjects met the CDC criteria for chronic fatigue syndrome.Discussion:It is argued that neurasthenia exists as a distinct diagnostic category,though it may roughly be labelled as undifferentiated somatoform disorder or chronic fatigue syndrome in accordance with Western diagnostic classifications.

应用CCMD—2、ICD—10、DSM—Ⅲ—R、CDC诊断标准定式评估96例非器质性慢性疲劳为主诉的门诊病人,以比较不同分类诊断系统的差异。结果发现,26例病人诊断一致性较好。其余70例符合CCMD—2神经衰弱诊断标准的病人,应用ICD—10标准,62例诊断为神经衰弱,8例诊断为未分化性躯体形式障碍(USD);应用DSM—Ⅲ—R标准,58例符合USD诊断,12例无诊断;应用CDC标准,13例符合慢性疲劳综合征(CFS)诊断。据此作者认为,作为一个疾病单元,神经衰弱并未消失,可能等同于目前欧美诊断系统中的USD和CFS。

Chronic Fatigue Syndrome(CFS) is a new disease presented in recent years. 32 cases of CFS were treated with modified Qinshuyiqi decoction of Dong yuan. There were obvious theraputical effects on fatigue, low fever, ache, narcolepsy, swelling of lymph and sweating. The total effective rate was over 80%.

慢性疲劳综合征(CFS)是近几年提出的一种新疾病,本文运用东垣清暑益气治疗慢性疲劳综合征32例。对CFS症状群中的疲劳、低热、肌肉酸痛、嗜睡、淋巴结肿大,汗出较多均有明显的治疗效果,总有效率均达80%以上。

The mechnism of exercise-induced fatigue(FIF)and the effect of "Tifukang" Recipe(TFK)on neuropeptide Y(NPY)of brainin rat with EIF were studied.Avidin-biotin-complex immunohistochemistry and image pattern analysis were used to observe the effect of TEK on the changes of NPY in different areas of brain.Results:After long term(7 weeks)and high intensity exercise (from15m/min gradually increased to 35m/min,20 to 25min/d),at rest,NPY was unchanged in paraventricularthalamic (PV),dorso- medial hypothalamic(DM)nucleus...

The mechnism of exercise-induced fatigue(FIF)and the effect of "Tifukang" Recipe(TFK)on neuropeptide Y(NPY)of brainin rat with EIF were studied.Avidin-biotin-complex immunohistochemistry and image pattern analysis were used to observe the effect of TEK on the changes of NPY in different areas of brain.Results:After long term(7 weeks)and high intensity exercise (from15m/min gradually increased to 35m/min,20 to 25min/d),at rest,NPY was unchanged in paraventricularthalamic (PV),dorso- medial hypothalamic(DM)nucleus and ventromedial hypothalamic (VMH)nuclues,however,at 3 houre postexercise after final ex- ercise it changed obviously.NPY was not remarkably affected in the chronic fatigue stress by TFk,but it was regulated bidirection- ally by TFk,expecially at 3 hours post-exercise in the acute fatigue stress;down in PV and DM,while up in VMH.Conclusion;The dynamic changes of NPY in different areas of brain in rat with EIF were different,and TFK had bidirectionally regulation effect on NPY.

为了从脑组织神经肽Y水平探索运动性疲劳产生及药物调节的机理,采用ABC免疫组织化学法结合图象分析,观察中药复方“体复康”对运动性疲劳大鼠不同脑区神经肽Y的动态变化。结果显示:一定强度(速度由15m/min递增至35m/min,运动时间为20至25min/d)长期(共7周)运动的大鼠,安静状态下在丘脑室旁核(PV)、下丘脑背内侧核(DM)、下丘脑腹内侧核(VMH)等核团NPY无显著性变化;在此基础上的未次急性运动结束后3小时NPY变化尤为明显。中药复方“体复康”对于慢性运动应激过程中NPY的调节作用不明显,在急性运动应激中,中药复方对NPY的调节在运动结束后3小时尤为明显,且呈双向调节,即在PV、DM核团下调,而在VMH核团上调,结论:神经肽Y在运动性疲劳大鼠不同脑区呈现不同的动态变化趋势,中药复方“体复康”对NPY有双向调节作用。
