Artificial hydroxyapatite (HAP) combined with autologous red bone marrow (BM) was used to repair bone defects caused by the innocent bone tumor curetted in five patients. The method is a sample and safe manipulation, which free from toxic reaction, causes less trauma, thus, a satisfactory recovery results.

Objective To observe the effect of repairing segmental bony defects with the composite grafting of autologous peripheral blood stem cell(APBSC) and decalcified bone matrix(DBM) . Methods 36 rabbits were divided into 3 groups randomly. On the average, 12 rabbits in each group - DBM; autologous red bone marrow(ARBM/DBM);and APBSC/DBM. A segmental bone defectdcm in the length)was created at two radii in each rabbit. The curative effect of segmental bone defects was assessed by serial radiographic, histologic studies and biomechanical test respectively. Results APBSC/DBM was superior to DBM, and similar to ARBM/DBM in X-ray, modified Gary' s radiographic scoring and microscope test in the second, fourth, eighth, and fourteenth week after operation; APBSC/DBM was also superior to DBM in the load broken of intact bone and the scoring histomorphology in bone defects repairing in the fourteenth week after operatrion. But compared with ARBM/DBM, there was not a significant difference in the load broken of intact bone, and was in the scoring histomorphology in bone defects repairing. Conclusions The composite grafting of APBSC/DBM is superior to DBM alone, and similar to the composite grafting of ARBM/DBM in the curative effect of bone defects reparing. From the view of clinical practice, the methods may show a better prospect.

目的 观察自体外周血干细胞(Autologous Pexipheral Blood Stem Cell—APBSC)/脱钙骨(Decalcified Bone Matrix-DBM)复合移植治疗骨缺损的疗效。方法 36只家兔桡骨做成1cm骨缺损,随机分为DBM组自体红骨髓(Autologous red bone marrow—ARBM)/DBM组APBSC/DBM组,每组12只。分别进行X线观察、生物力学检查和组织学观察的对比研究。结果 术后第2、4、8、14周APBSE/DBM组X线观察改进的Gary评分和光镜观察均优于DBM组,与ARBM/DBM组相似,术后第14周APBSC/DBM组整骨破坏载荷和骨缺损修复,形态学评分也优于DBM组,与ARBM/DBM组比较,前者没有显著性差异,后者有显著性差异。结论 APBSC/DBM复合移植治疗骨缺损的疗效优于单纯DBM移植,与ARBM/DBM组相似。结合临床我们认为具有良好的应用前景。

To develop a new substitute of bone graft, which is composed of porcine bone morphogenetic protein (pBMP) and the magnetic porous tricalcium phosphate (MPTCP), we investigated the effects of pBMP/MPTCP (Group A), autologous red bone marrow/porus tricalcium phosphate (BM/PTCP) (Group B), pBMP/PTCP(Group C) on the repair of bone defects created surgically in rabbits . By gross inspection, roentgenography examination, histological examination, immunohistochemistry test of osteocalcim, quantitive histological measurement of new bone formation and biomechanical test, three kinds of substitutes were proved to be biocompatible. Group A induced more new bone formation than other two groups, and showed a better biomechanical function (P<0.01). It is believed that pBMP/MPTCP has characteristics of strong osteoinductive capacity, good biocompatibility, excellent filling of bone defects and gradual biodegradation.

将猪骨形态发生蛋白 (BMP)与磁性多孔磷酸三钙 (MPTCP)结合制成人工骨 ,手术造成兔桡骨中段 1cm缺损 ,分别嵌入pBMP/MPTCP(A组 ) ,自体红骨髓 /多孔磷酸三钙 (BM/PTCP) (B组 )及pBMP/PTCP(C组 )。观察术后一般情况、免疫球蛋白及补体、大体标本骨痂量、X线、组织形态学、骨钙素检测、新骨形成面积比、生物力学实验等指标。实验结果显示 ,三组骨缺损均有不同程度修复且生物相容性良好 ,但A组较另二组有更多新骨形成及更好的力学性能 (P <0 .0 1) ;B ,C二组间差异无显著性。作者认为pBMP/MPTCP有以下特点 :促进新骨形成能力强 ,生物相容性良好 ,有良好的填充作用 ,可在体内逐渐降解
