Thirty-two cases of exophthalmos with intra-orbital lesions were

在对眼球突出的32例病例进行电子计算机断层扫描(CT)中,发现主要病因为眶内原发性肿瘤,共24例;其次为Graves病及眶内炎症。通过介绍各类眶内占位性病变的CT所见,说明电子计算机断层扫描是诊断眶内占位性病变的最有效方法。

The pathologic and clinical features of 160 cases of intra-orbital tumors were analyzed. There were 27 types of tumors. The most common ones were pleomorphic adenoma, lymphoma, neurolemmoma and dermiod cyst. Most patients complained of exophthalmos. The recurrence of pleomorphic adenoma and neurofibroma is explained in view of their histopathology.

分析了160例眶内肿瘤的病理及部分临床特征,其类型达27种。病人多以眼突为主诉,恶性肿瘤(除恶性淋巴瘤外)发病早,病程短。最常见的眼眶肿瘤是泪腺混合瘤、血管瘤、淋巴瘤、神经鞘瘤、皮样囊肿等,并从组织学上解释了泪腺混合瘤、神经纤维瘤的易复发性。

笔者分析了40例眶内占位性病变的MRI表现并与CT比较。结果表明MRI对眶内占位性病变发现率极高,特别是对视神经的管内段和颅内蔓延或眶外软组织浸润范围的确定较CT更准确,其多方位扫描功能使定位诊断符合率高达100%,特别是冠状面对定位诊断的作用极大。据不同组织有不同的MRI信号强度变化的特点,结合形态学变化,MRI对一些病变可作出定性诊断。
