Objective:To study the effect of supine and prone position on lung mechanics in the preterm infants with pneumonia to find a suitable position to the neonates with pneumonia.Methods:Thirty non-inspired oxygen preterm infants and 18 inspired oxygen preterm infants with pneumonia had lung mechanics measured in the supine and prone position respectively with American Biocore CP-100 Neonatal Pulmonary Apparatus. Results:In the non-inspired oxygen groups, respiratory rate in the prone position was significantly lower...

Objective:To study the effect of supine and prone position on lung mechanics in the preterm infants with pneumonia to find a suitable position to the neonates with pneumonia.Methods:Thirty non-inspired oxygen preterm infants and 18 inspired oxygen preterm infants with pneumonia had lung mechanics measured in the supine and prone position respectively with American Biocore CP-100 Neonatal Pulmonary Apparatus. Results:In the non-inspired oxygen groups, respiratory rate in the prone position was significantly lower than in the supine(43.4±10.2 v 49.8±11.7);Tidal volume in the prone position was significantly higher than in the supine position(3.11±0.92 v 2.48±0.77 ml/kg); pulmonary resistance in the prone position was significantly lower than in the supine position(98.6±50.1 v 117.4±41.5cm H_2O/L/s,);dynamic pulmonary compliance in the prone position was significantly higher than in the supine position (1.115±0.401v 0.914±0.48ml/cm H_2O/kg);work of breathing in the prone position was significantly higher than in the supine position(8.0±3.8 v 5.9±2.8gmcm/kg). In the inspired oxygen groups, respiratory rate in the prone position was significantly lower than in the supine(52.7±10.2 v 56.1±12.4);Tidal volume in the prone position was significantly higher than in the supine position(2.13±0.42 v 1.79±0.66 ml/kg); pulmonary resistance in the prone position was significantly lower than in the supine position(290.4±107.0 v 319.1±104.1cm H_2O/L/s);dynamic pulmonary compliance in the prone position was significantly higher than in the supine position (0.562±0.192 v 0.488±0.217ml/cm H_2O/kg);work of breathing in the prone position was significantly higher than in the supine position(10.1±4.7 v 7.3±3.4gm cm/kg).Conclusion: Either the non-inspired oxygen groups or the inspired oxygen groups, the prone position may not onlyimprove the tidal volume dynamic pulmonary compliance and work of breathing ,but also decrease respiratory rate and pulmonary resistance. It suggests that the prone position is the suitable position to the preterm infants with pneumonia.
