The need for a reliable method of estimating depth of anaesthesia has existed since the introduction of anesthesia. This paper presents a new approach to predict response during isoflurane anaesthesia by using mutual infomation time series of electroencephalograms and their complexity analysis. The need for a reliable method of estimating depth of anaesthesia has existed since the introduction of anesthesia. This paper presents a new approach to predict response during isoflurane anaesthesia by using mutual infomation time series of electroencephalograms and their complexity analysis. The mutual infomation time series between four leads were first computed using the EEG time series. The Lempel Ziv complexity measures, C(n)s, were extracted from the mutual infomation time series by complexity analysis. Prediction was made by means of neural network(ANN). The input to the neural network was the C(n) values and the MAC level, the output was results of the prediction. From 98 consenting patient experiments, 98 distinct EEG recordings were collected prior to incision during isoflurane anaesthesia of different levels. Hemodynamic variables and respiration pattern were also monitored. After skin incision, each patient was observed carefully for 2 min to detect purposeful responses and then the EEG was labelled as 0.0 for responder or as 1.0 for non-responder. Training and testing the ANN used the drop one person method, response prediction was tested by monitoring the response to incision and the result given by the ANN. The system was able to correctly classify subsequent response with an average accuracy of 91.84%. The results showed that the method has a better performance than others, such as spectral edge frequency, median frequency, and bispectral analysis. This method is computationally fast and acceptable real time clinical performance could be obtained.

自从麻醉应用于临床以来 ,麻醉深度的可靠监测是十分必要的。但到目前 ,尚没有一个公认可靠准确的方法。本文提出一种麻醉深度监测的新方法 ,即用脑电的互信息序列及其复杂度分析来反应异氟醚麻醉条件下患者的麻醉情况。首先计算出四导脑电的互信息时间序列 ,然后计算该序列的复杂性测度 ,借助于神经网络可实现用脑电来监测麻醉深度。神经网络的输入是复杂度值和对应的MAC水平 ,输出即是麻醉深度状况的结果。从 98个自愿患者进行的实验中得到 98个不同程度异氟醚麻醉时切皮前脑电片断 ,同时监测血液动力学参数和患者的呼吸模式。切皮后 ,仔细观察每个患者两分钟 ,以检查患者对切皮的反应 ,把有反应时的脑电标上 0 .0 ,无反应时的脑电标上 1.0。训练和测试神经网络用“去掉一个”方法。从患者对切皮的反应和神经网络的输出结果可检测系统的预测情况。实验表明 ,系统对切皮后患者反应的平均正确判断率为 91.84 % ,该方法比传统脑电分析方法如边缘频率法、中心频率法、双谱分析法有更高的准确性。另外 ,该方法计算时间短 ,适合临床实时使用。

Methods: Forty patients scheduled for selective abdominal operation under general anaesthesia were divided into the elderly group or non-elderly group(n=20)according to their age, average (68±9) years and (42±6) years respectively. The patients received 0.05~0.2 mg/(kg·min) remifentanyl-2~3 mg/(kg·h) propofol-1.5%~2.5% isoflurane...

Objective:To compare the difference of recovery time and recovery quality in the elderly and non-elderly patients after remifentanyl-propofol-isoflurane anaesthesia. Methods: Forty patients scheduled for selective abdominal operation under general anaesthesia were divided into the elderly group or non-elderly group(n=20)according to their age, average (68±9) years and (42±6) years respectively. The patients received 0.05~0.2 mg/(kg·min) remifentanyl-2~3 mg/(kg·h) propofol-1.5%~2.5% isoflurane general anaesthesia after induction and tracheal intubation, and rocuronium were given for muscle relaxation. The recovery and extubation time and complication related to recovery and extubation were recorded. Results: The patient′s demographics, operation time and drug consumption of two groups were similar. The recovery time of the elderly group and non-elderly group were(8.5±4.4)min, (7.9±4.2)min,and the extubation time of the elderly group and non-elderly group were(12.3±5.0)min, (11.6±6.4)min respectively. Two cases(9.5%)showed agitation in the non-elderly while no patient showed agitation in the elderly group. No nausea/vomiting, respiratory depression and un awareness were found for all patients. Conclusion: Age had no influence on the recovery time and recovery quality for the elderly and non-elderly patient effects after remifentanyl-propofol-isoflurane anaesthesia.

目的 :比较老年和非老年患者对雷米芬太尼 丙泊酚 异氟醚麻醉后苏醒时间及质量的差异。方法 :选择全麻下择期腹部手术患者 4 0例 ,根据年龄分为老年 (6 8± 7.0 )岁和非老年组 (4 0± 8.3)岁 ,每组 2 0例。静脉诱导气管插管后采用丙泊酚 2~ 3mg/ (kg·h) 雷米芬太尼 0 .0 5~ 0 .2 μg/ (kg·min) 异氟醚 1.5 %~ 2 .5 %维持麻醉 ,罗库溴铵维持肌松。记录患者苏醒时间以及苏醒期间患者的反应。结果 :两组患者一般状况、手术时间、术中麻醉药物用量无显著性差异 ;老年组和非老年组苏醒时间分别为 (8.5± 4 .4 )min、(7.9± 4 .2 )min ,拔管时间分别为 (12 .3± 5 .0 )min、(11.6± 6 .4 )min ;恢复期老年、非老年组分别出现躁动 0例、2例 (9.5 % ) ;两组均无恶心、呕吐、呼吸抑制等并发症 ,对拔管刺激无知晓。结论 :年龄对异氟醚 丙泊酚 雷米芬太尼麻醉时间及苏醒质量无明显影响。

The time duration to opening eyes on command and the time for extubation after surgery were similar between the two groups. Conclusion: Remifentanil was as effective as epidural block in providing analgesia and suppressing hemodynamic responses to thoracic surgery under isoflurane anaesthesia. But under Remifentanil anaesthesia the hemodynamic change was stable and depth of anesthesia was more preferable.

目的:比较胸科手术中瑞芬太尼全麻与硬膜外阻滞复合全麻围术期血流动力学的变化、麻醉深度及术毕清醒情况。方法:60例择期行开胸手术患者,随机分为两组,瑞芬太尼全麻组(R组)异氟醚吸入复合静脉泵注瑞芬太尼全麻组;硬膜外复合全麻组(E组)异氟醚吸入全麻复合胸段硬膜外阻滞组。记录两组患者围术期BP、HR及脑电双频谱指数(BIS)、清醒时间的变化。结果:R组病人围术期的血压、心率稳定,E组病人血压下降较大,在麻醉维持期间R组BIS显著低于E组,两组患者从手术结束到呼之睁眼和拔除气管导管时间相似。结论:瑞芬太尼全麻与硬膜外复合全麻在胸科手术中都能提供有效的镇痛和抑制手术刺激引起的血液动力学反应,但瑞芬太尼全麻术中不但血流动力学稳定而且麻醉深度更佳。
