Serum troponin (cTnI) and coxsackie B virus(CBV) IgM?IgG were measured dynamically in VM patients (VM,n=102),virus infection patients (VI, n=41)and normal subjects (n=46) with solid phase immunity chromatography and ELISA sandwich assay respectively. The results indicated:①The positire rate of cTnI was much higher than that of creatine kinase, CK MB and LDH among VM patients (70.58% vs 15.69%,P<0.01).The positive rate of cTnI reached 87.93% in the acute VM patients . No one was abnormal for cTnI and CK,CK MB,LDH in VI and normal subjects. ② The positive rate of CBV IgM in VM was higher than that in VI (67.65% vs 17.7%,P<0.05). ③ AmongVM patients ,the abnormal rate of cTnI was markedly higher in IgM positive patients than in IgG positive patients (73.91% vs 43.75%,P<0.05). On the other hand,the abnormalities of IgM were much more than that of IgG among the cTnI positive patients (83.33% vs 41.67%,P<0.05) and the abnormalities of IgM were less than that of IgG among the cTnI negative patients (30.0% vs 60.0% , P<0.05). These suggested that the sensitivity , specificity and accuratity of cTnI to diagnose myocardial cell injury were much superiorer to that of CK ,CK MB and LDH CBV IgM,IgG were helpful to assess the causes and periods of VM. It is more simple and mature to combine cTnI and CBV Ig to diagnose VM.

Objective: To investigate the clinical significance of plasma catecholamine (CA) and cardiac troponin I (cTnI) between virus myocarditis (VM) and virus infection (VI). Methods: Plasma CA and cTnI were measured in 46 virus myocarditis (VM) patients. Thirty-five VI patients and 30 normal subjects with high speed liquid chromatography and solid phase immunity chromatography respectively. Results: (1) Plasma noradrenalin level was much higher in VM and VI groups than that in control group (P < 0.01). The noradrenalin was markedly higher in VM group than that in VI group (P < 0.01). (2) The positive rate of cTnl was significantly higher than that of usual myocardiac enzyme (creatine kinase, CKMB and LDH) in VM group (P<0.05). The accurate rate of cTnl was much superior to that of usual myocardiac enzyme (P< 0.05). Conclusion: cTnl is much better than usual myocardiac enzyme diagnosing myocardial injury. The stressing in VM group is much greater than that in VI group. Combining CA with cTnl will be helpful for differential diagnosis of VM and VI.

目的:探讨血浆儿茶酚胺(CA)与肌钙蛋白I(cTnI)水平对病毒性心肌炎(VM)与病毒感染(VI)的临床意义。方法:分别用高效液相色谱法和固相免疫层析法测VM组(n=46)、Ⅵ组(n=35)和对照组(n=30)的血浆CA和cTnI。结果:(1)VM与VI组的去甲肾上腺素(NE)值明显高于对照组(P<0.01),并且VM组的NE值也明显高于Ⅵ组(P<0.01)。(2)VM组cTnI阳性率明显高于普通心肌酶阳性率(P<0.05),Ⅵ组与对照组无1例cTnI阳性。cTnI的准确率明显优于普通心肌酶(P<0.05)。结论:cTnI对VM的心肌损伤具有良好的敏感性、特异性和准确性,明显优于普通心肌酶;VM患者其应激状态、交感兴奋程度明显高于VI患者;联合CA与cTnI两指标有助于VM与VI的鉴别诊断。
