本文分析了两例小儿慢性粒细胞性白血病,一例为成人型,另一例为婴儿型。明显脾大,白血胞增多,血小板增多,碱性磷酸酶积分降低,及Ph~1染色体阳性为成人型慢性粒细胞白血病特点;皮疹,易于感染,抗碱血红蛋白增多及Ph~1染色阴性为婴儿型慢性粒细胞白血病特点。有人曾把小儿慢性粒细胞性白血病分为少年型、婴儿型与家族性三种,并认为这都不同于成人型慢性粒细胞性白血病。作者等不同意这样划分,我们认为,慢性粒细胞性白血病只分为成人型与婴儿型两种,成人型慢性粒细胞性白血病也可见于小儿,至于家族型是否真正成为?个型,目前还不能肯定。

The plasma level of von Willebrand factor(vWf) and fibronectin(Fn) in 15 cases of chronic myeloid leukemia(CML) and 5 cases of acute transfcrmation was studied. The increase of vWf and the decrease of Fn was found significantly in CML, especially in acute transformation cases,Thus, the plasma von Willebrand Factor and Fibronectin determinatin may be used as the parameter for the severity in CML, especially for acute transformation.

All patients (17 AML and 13 CML) demonstrated impaired autologous mixed lymphocyte reaction (p<0.001) as well as a markedly decreased proportion of MAC-120- and HLA-DQ-positive monocytes...

The autologous mixed lymphocyte reaction was measured with isotope incorporation technic and expression of HLA class II antigens on peripheral blood inonocytes was analyzed by indirect immunofluorescence assay with monoclonal antibodies Tu22, Tu36 and Anti-Leu-M2 in patients with acute and chronic myeloid leukemia (AML, CML). All patients (17 AML and 13 CML) demonstrated impaired autologous mixed lymphocyte reaction (p<0.001) as well as a markedly decreased proportion of MAC-120- and HLA-DQ-positive monocytes as compared with controls (p<0.001), while the percentage of HLA-DR-positive monocytes in the patients was unchanged (p>0.05). The controls in these studies were the patients' HLA identical normal siblings exclusively. The results suggested that impaired autologous mixed lymphocyte reaction was correlated with the decreased expression of HLA-DQ antigen on monocytes in patients with acute and chronic myeloid leukemia.

T细胞在自身非T细胞刺激下发生增殖反应称为自身混合淋巴细胞反应(AMLR)。它反映机体免疫系统内的功能调节机制。本文以氚标胸腺嘧啶(~3H-TdR)掺入的淋巴细胞转化试验测定单核细胞(Mon)刺激自身T细胞的增殖反应;用单克隆抗体(McAb)Tu22、Tu36和 Anti-Leu-M,通过间接免疫荧光技术测定 Mon表面 HLA-Ⅱ类抗原的表达,分析急、慢性髓细胞白血病(AML 和CML)患者 AMLR 的改变及其机理。结果表明,与 HLA 全相同的同胞比较,患者 AMLR 明显减弱(p<0.001);患者Tu22~+(HLA-DQ~+)Mon 百分率明显下降(p<0.001),但Tu36~+(HLA-DR~+)Mon 百分率无明显改变(P>0.05)。提示髓细胞白血病患者 AMLR改变与刺激细胞(Mon)表面HLA-DQ 抗原表达障碍有关。
