Objective To evaluate CT 3D imaging in observing spinal internal fixation with metal fixer.Materials and Methods CT thin slice scanning was performed in 70 patients after spinal internal fixation with metal fixer. 3D reformation, including multiplanar reconstruction (MPR), surface shaded display (SSD) and volume rendering (VR), was made. The displaying effect of the spine and internal fixer was evaluated. Results On MPR, the fixer and its relation with the surrounding structures were well demonstrated, although significant artifacts were seen. The artifacts became less when the MPR thickness was appropriately increased. The optimal VR images clearly showed the metal fixer and its surrounding structures.Conclusion The use of CT 3D reformation technique in a reasonable way can fully show the internal metal fixer as well as its relationship with the surrounding structures.

目的 探讨CT三维成像在脊柱金属内固定术后评价中的应用技术。资料与方法 对 70例脊柱金属内固定术后患者行螺旋CT薄层扫描和三维成像 ,包括多平面重建 (MPR)、三维表面遮盖显示 (3D SSD)、容积重建(VR) ,并评价其对内固定物及脊柱结构的显示效果。结果 MPR多方位成像清楚显示固定物及其关系 ,但有明显金属伪影 ,适当增加MPR的厚度 ,伪影减少 ,骨和金属物的分辨率提高 ;VR清楚地显示固定物与周围结构的立体关系 ,基本无金属伪影 ,恰当的VR模式最佳显示了金属固定物及其相互关系。结论 合理使用CT三维重建技术可以充分展示脊柱金属固定物全程结构及其相互关系。

Objective: To investigate the imaging methods of multi-slice spiral CT(MSCT) with volume rendering technique(VRT) 3D renconstruction, and evaluate its clinical application in the diagnosis of spinal trauma. Material and Methods: There were 65 cases in this group. MSCT with volume rendering were performed in all cases. Then thin slice low and high constrast image were reconstructed. Objective: To investigate the imaging methods of multi-slice spiral CT(MSCT) with volume rendering technique(VRT) 3D renconstruction, and evaluate its clinical application in the diagnosis of spinal trauma. Material and Methods: There were 65 cases in this group. MSCT with volume rendering were performed in all cases. Then thin slice low and high constrast image were reconstructed. After that, all the source images were input into a 3D work station. High quality VRT-3D images of spinal trauma and after spinal internal fixation were obtained by selecting appropriate CT value. All the pictures were observed and analysed by two radiologist. Results: The optimal VRT images clearly showed alignment and displacement of fracture, and displayed space changes of spinal trauma. In observing spinal internal fixation with metal fixer, the optimal VRT images clearly showed the metal fixer and its surrounding structures. Conclusion: VRT-3D images of MSCT can directly, stereoscopically and multi-angularly show spinal trauma and after spinal internal fixation for the surgeons to decide clinical management.

目的:探讨多层螺旋CT容积漫游技术(VRT)三维重建成像方法,评价其在脊柱损伤诊断的价值。方法:对65例脊柱损伤患者进行CT容积扫描,将原始图像进行低对比及高对比薄层重建,然后将重建图像导入CT3D工作站,选择合适CT值范围、不透明度、明亮度对脊柱损伤及术后病例脊柱进行VRT成像。并由2位CT诊断医生共同对相应图像进行观察分析。结果:VRT图像可清楚显示脊柱骨折移位、骨折线走向及脊柱损伤的三维空间关系;对内固定术后脊柱,VRT图像能清楚显示固定物与周围结构立体关系,基本无金属伪影。结论:多层螺旋CT-VRT后处理成像能清楚、立体、直观、多方位地显示脊柱损伤及术后改变,可为临床医生治疗及随访提供依据。
