On the basis of utilizing free skin graft with subdcrmal vascular network, the author began to apply subdermal vascular network skin flap(SVNSF) in repairing skin defects of face, neck and limbs in 1983.66 cases were all successful, The advantage of SVNSF is that the subcutaneous fat layer of the flap is largely removed, subdermal vascular network plane iswholly or partially exposed, so that the early circulation is reestablished beween the flap and the recipient bed and hence the flap has dual vascular supply including the pedicle, it brcaks the limits of traditional flap about the ratio of length to width and the time of pedicle off. Because of soft-touch and aesthetical-vision, SVNSF is a more ideal flap to reconstruct the skin defects of face, neck and limbs.

作者在开展真皮下血管网皮片移植的基础上,1983年开始应用真皮下血管网皮皮瓣修复面,颈及四肢等部位皮肤缺损创面,66例全部成功。这种皮瓣的特点是将普通皮瓣的皮下脂肪大部份去除,部份或全部地暴露真皮下血管网层,以便早期与受区建立血供联系,与蒂部共同形成两个血供渠道,因而在长宽比例及断蒂时间方面突破了传统皮瓣的界限。由于质地柔软,颜色美观,是修复面颈部及四肢皮肤缺损较为理想的一种皮瓣。

目的 研究真皮下血管网薄皮瓣在手部软组织缺损修复中的应用。方法 对因外伤、热电等烧伤引起的手部软组织缺损或瘢痕挛缩畸形23 例( 皮瓣面积为16 cm ×5 cm ~8 cm ×4 cm) 患者,采用真皮下血管网皮瓣修复。结果 断蒂时间最短为术后7 d ,最长为术后12 d ,23 例患者均获得满意效果。结论 该薄皮瓣在各种原因引起的手部软组织缺损修复中有其明显优点。

目的探讨真皮下血管网皮瓣的切取方法及修复皮肤软组织缺损的手术效果。方法临床应用真皮下血管网皮瓣修复手部创面32例,皮瓣面积最大14.0cm×10.0cm,最小2.0cm×1.2cm。断蒂时间为术后5~7天。结果本组32例中的30例皮瓣一期愈合。结论(1)真皮下血管网皮瓣血供重建速度快、成活面积大;(2)断蒂时间以术后第6天为宜;(3)本组应用的耕耘开沟式平行分离脂肪法,利于真皮下血管网的保护,提高皮瓣的成活质量;(4)皮瓣质地良好,是修复手创面的较理想材料。
