Objective: Recent studies have shown that cell implantation can replace infarcted myocardium to improve cardiac performance. The present study was designed to investigate the effects of autologous bone marrow mononuclear cell (BM-MNC) implantation into myocardium bordering the infarction with or without induced by 5-azacytidine on cardiac function after acute myocardial infarction (AMI) in rabbits. Methods: AMI was replicated by ligating the left anterior descending coronary artery. Rabbits were randomly divided into three groups: (1) BM -MNC induced by 5-azacytidine implantation (n=7), (2) BM-MNC implantation alo ne (n=7), and (3) AMI control (n=7). In addition, sham-operated (n=5) rabbit s were randomly selected to serve as non-infarction control. Animals for cell im p lantation were received intramyocardial injection of autologous BM-MNC in myoca rdium bordering the infarction, and echocardiography and hemodynamic studies wer e performed to evaluate cardiac function following 28 days of implantation. Results: Compared with the sham-operated group, the left ventricle (LV) end diastolic pressure (LVEDP) was significantly increased (P<0.05) in all AMI procedure groups. The ΔD (Dd minus Ds) to Dd ratio (ΔD/Dd), wall thickness, ejection fraction (EF), left ventricle systolic pressure (LVSP) and maximum LV pressure rising rates (dp/dtmax) were all significantly reduced (all P< 0.05). These changes indicated LV remodeling occurred and LV function were impaired. Comparison among all AMI procedure groups showed that impaired LV function were improved by autologous BM-MNC implantation, as revealed by decreased LVEDP (BM-MNC implantation vs. AMI alone, P<0.05) and increased ΔD/Dd, wall thickness, EF, LVSP, and dp/dt max (BM-MNC implantation vs. AMI alone, all P<0.05). Furthermore, BM-MNC induced by 5-azacytidine implantation did not confer a further improvement of cardiac function with non-significant changes of all assessed parameters when compared with BM-MNC implantation alone (all P>0.05). Conclusion: BM-MNC induced by 5-azacytidine implantation into myocardium bordering the infarction can significantly improve impaired cardiac function associated with LV remodeling after AMI, however such improvement is not further promoted compared with that in BM-MNC implantation group alone.

Methods: 27 adults suffered from ischemic heart failure were enrolled in this study. Bone marrow mononuclear cells were isolated by Ficoll density gradient centrifugation from the marrow blood obtained from the patients. Results: These cells were injected into coronary artery through a peripheral artery puncture. A 3-month follow-up was performed to evaluate the clinical symptom, NYHA classification, and re-hospitalization because of cardiovascular events after the treatment. Conclusion:Cellular cardiomyoplasty through autologous bone marrow mononuclear cell transplantation is safe and feasible. The results show that the treatment improved the clinical status of the patients and ischemic stress of the local cardiomyocyte.

Objective To examine whether the in vivo implantation of autologous bone-marrow mononuclear cells (BM-MNCs) can augment postnatal neovascularization in rat model of hind limb ischemia.Methods Rat BM-MNCs were isolated by centrifugation through a Ficoll-paque density gradient. The rat ischemic hind limb model was made by ligation of the right femoral artery and its branches of imbred Wistar rats. BM-MNCs (MNC group) or phosphate saline buffer (PBS group) were injected into 7 points of the ischemic muscles (20 μl/point). To assess angiogenesis, histologic evaluation was performed on tissue slices from adductor muscles with immunohistochemical staining at the 2nd and 4th weeks after the ligation. Severity of ischemic insult was evaluated by the femoral arteriovenous oxgen difference (AVDO 2) at the 2nd and 4th weeks after the ligation.Results The MNC groups had a higher capillary density and higher capillary/muscle fiber ratios than that of the sham and PBS groups(P<0.01)at the 2nd and 4th weeks after the ligation. AVDO 2 of the MNC group significantly decreased compared with that of the PBS groups(P<0.05).Conclusion The implantation of autologous BM-MNCs induced neovascularization in a rat ischemic hind limb model. The implantation of autologous BM-MNCs augmented the collateral perfusion of ischemic hind limb.

目的 :研究骨髓单个核细胞局部移植对缺血组织血运重建的影响。方法 :从大鼠胫骨及股骨采集骨髓细胞 ,密度梯度离心法分离出骨髓单个核细胞。分离、切断近交系Wistar大鼠右侧股动脉及其分支 ,建立单侧后肢缺血模型。分 7点在缺血后肢局部注射单个核细胞悬液 (MNC组 )或磷酸盐缓冲液 (PBS组 )。术后第 2 ,4周测定各组后肢动静脉氧差 (AVDO2 )并取内收肌制作组织切片 ,免疫组织化学法分析血管新生。结果 :术后第 2 ,4周MNC组内收肌毛细血管密度、毛细血管与肌纤维比值高于PBS组 (P <0 .0 1) ,AVDO2 低于同期PBS组 (P <0 .0 5 )。结论 :在大鼠缺血后肢局部直接植入骨髓单个核细胞可诱导新生血管形成 ,改善缺血组织的血流灌注
