Methods: Thirty New Zealand rabbits were randomly assigned into 10 groups(n=3), and were made a 5mm diameter full;thickness defect on each hemi mandibular body. All the animals were sacrificed at postoperative 2, 4, 6, 8 and 12 weeks respectively, then they were X;rayed the bone defects and recorded the relative bone density(RBD). The status of bone repair between two groups was compared. Results: From the X;ray, at every stage of bone repair, the HGF group was superior to the control group. In aspect of RBD,there was significant difference between the two groups (P<0.05); it was higher in HGF group than in the control group at the 2nd and 4th week(P<0.05), but not significantly different at the 6th, 8th and 12th week in the two groups(P>0.05). Conclusion: pUDKH has an enhancing role on mandibular defects repair, so HGF gene therapy has great therapeutic potential for bone defects repair.

目的:探讨肝细胞生长因子(HGF)基因治疗对下颌骨缺损修复的促进作用。方法:新西兰大耳白兔30只,平均分为10组,在兔下颌骨体部造成直径5mm圆形贯通性骨缺损,其中5组将含有人HGF全长cDNA片段的表达型质粒-pUDKH按照每只250μg的剂量直接应用于骨缺损局部,另外5组应用等剂量空质粒pUDK作为对照。于手术后第2、4、6、8、12周分批处死两组动物,拍摄骨缺损区的X线片,并记录X线片的相对骨密度值,比较两组骨缺损愈合的情况。结果:手术后X线片示,HGF组优于对照组。两组间相对骨密度(RBD)差异显著(P<0.05)。手术后第2、4周HGF组高于对照组(P<0.05)熏第6、8、12周两组间差异无统计学意义(P>0.05)。结论:pUDKH对下颌骨缺损的修复具有促进作用,HGF基因治疗有望用于促进骨缺损的修复。

Methods The subjects were 10 dogs which is use for animal model of cranial, mandibular defect repaired by tissue engineering bone.All dogs were underwent 3d-CT and make three-dimensional geometric model.Virtual osteotomy were made to get cranial,mandibular bone defect and the data were input into rapid prototyping machine to get three-dimensional model.The sacrificial mould was made and was the filled with TCP and sintered to produce the bone scaffold.The bone defects were repaired with the cell-scaffold construct.All cases were followed up by 3d-CT post-operation. Results Tissue engineering bone scaffold which were prefabricated by the substantial model could match the craniomaxillofacial bone defects well. Conclusion Tissue engineering bone scaffolds could be fabricated based on computer aided design and rapid prototyping technique. 目的:采用快速成型技术对组织工程支架材料进行预塑形,探讨其在治疗颅颌面骨缺损中的应用前景。方法:选择10只成年杂交犬,均拟行组织工程骨修复颅颌面骨缺损实验研究(下颌骨节段缺损,颅骨全层缺损),螺旋CT连续薄层容积扫描,数据传至工作站后行三维重建,按实验设计方案进行模拟手术,并转换成*.stl文件格式,快速成型机制造骨缺损三维实体模型,据此制作阴模,填充磷酸三钙高温烧结后制成多孔支架,复合骨髓基质细胞后回植,螺旋CT检测组织工程骨和骨缺损完全匹配情况。结果:10例均按预设计制备骨缺损三维实体模型,据此制备个性化组织工程支架材料,手术回植和骨缺损完全匹配。结论:采用快速成型技术可对组织工程骨支架材料进行预塑形,对未来组织工程技术治疗复杂性颅颌面骨缺损具有重要的临床实用价值。
