Objective To study the effects of sitting position on pulmonary hemodynamics and intrapulmonary shunt (Qs/Qt) during neurosurgery. Methods Twenty eight patients scheduled for posterior fossa and posterior cervical spinal surgery, were anesthetized with sevoflurane fentanyl pancuronium or enflurane fentanyl pancuronium after intravenous induction. Pulmonary hemodynamics were monitored with Swan Ganz catheter technique and Qs/Qt was calculated from the data of arterial and mixture venous blood gas analysis, including the following parameters: CI, RAP, MPAP, PCWP, PVRI, pHa, PaCO 2, PaO 2, PaO 2/FiO 2 and Qs/Qt. The detection time points were as the follow: before anesthesia induction, before sitting position, immediately ,60 min and 120min after sitting position, and at the supine position after termination of surgery.Results After induction of anesthesia and during operation at sitting position, CI decreased significantly (P<0.01) except at the supine position after surgery. There were on changes in MPAP and PCWP, but PVRI increased significantly during sitting position (P<0.05-0.01). During anesthesia and operation, PaO 2 and SaO 2 were kept at high level (P<0.01), whereas there were no changes in arterial pH, PaCO 2, Qs/Qt and PaO 2/FiO 2 (P>0.05).Conclusions During neurosurgical procedure under general anesthesia and sitting position, pulmonary vascular auto regulation can be preserved and pulmonary oxygenation can be kept in normal.

目的 观测神经外科手术中坐位全身麻醉对肺血液动力及肺内分流 (Qs/Qt)的影响。方法  2 8例后颅窝及后颈髓手术病人 ,采用静脉复合诱导、吸入七氟醚或安氟醚及伍用芬太尼维持麻醉 ,以Swan Ganz导管和动脉、混合静脉血气检测方法监测肺血液动力学和肺内分流状况 ,参数包括CI、RAP、MPAP、PCWP、PVRI、pHa、PaCO2 、PaO2 、PaO2 /FiO2 及Qs/Qt。分别在麻醉前、坐位前、坐位后、坐位 6 0min、坐位 12 0min及术毕平卧位等时间点进行测定。结果 麻醉诱导后及坐位手术过程中 ,CI一直低于麻醉前值 (P <0 .0 1) ,术毕平卧后恢复到麻醉前水平 ;MPAP、PCWP无明显变化 ,PVRI一直明显增高 (P <0 .0 5~ 0 .0 1)。麻醉手术期间 ,血液的 pH和PaCO2 无明显变化 ,PaO2 和SaO2 在麻醉诱导后一直维持在高水平 (P <0 .0 1) ,Qs/Qt和PaO2 /FiO2 未见明显改变 (P >0 .0 5 )。结论 神经外科手术坐位全身麻醉期间 ,肺血管能保持自主调节功能 ,维持肺氧合功能在正常水平

Objective To study the effects of sitting position on systemic hemodynamics and oxygen metabolism during anesthesia and neurosurgery.Methods 28 patients scheduled for posterior fossa and posterior cervical spinal surgery were anesthetized with sevoflurane fentanyl pancuronium or enflurane fentanyl pancuronium.Swan Ganz catheter technique and arterial and mixture venous blood gas analysis were used to obtain hemodynamics and blood gas data and Fick equation were used to calculate DO 2 and VO 2.The detection time points were as follows:before anesthesia induction,before sitting position, after sitting position,at sitting 60min,sitting 120min and at the supine position after completion of surgery.Results After induction of anesthesia and during operation in sitting position,CI decreased under the preanesthesia value ( P< 0 001) with RAP increasing and MAP decreasing slightly,and SVRI raising persistently.DO 2 and VO 2 were significanly decreased ( P< 0 05~0 001),but no marked changes in PaCO 2,PvCO 2,pHa and HCO - 3 were found during the period.At the supine position after surgery,CI,SVRI,T,Hct,DO 2,VO 2 and PaCO 2 were returned or maintained at the preanesthesia values.However,pHa,pHv and BEb decreased and PvCO 2 increased significantly ( P< 0 05).Conclusion During neurosurgical procedure under general anesthesia with sitting position,global oxygen delivery and consumption can be kept well as long as the hemodynamics are properly maintained.Tissue oxygenation and metabolism may tend to imbalance during the postanesthetic recovery period.

Objective:To investigate the effect of propofol enflurane or propofol sevoflurane combining anaesthesia on liver function. Methods:Thirty two adult patients,scheduled for selective non hepatic surgery under general anaesthesia,were randomly allocated to receiving enflurane fentanyl (Group A, n =8),sevoflurane fentanyl (Group B, n =8),propofol enflurane fentanyl (Group C, n =8) and propofol sevoflurane fentanyl (Group D, n =8) ,respectively,to maintain anaesthesia . Group A and Group B served as control groups. From the blood were taken specimens all thirty two patients, before aesthesia, 2 h after induction and 3 d after operation to measure the serum concentration of ALT, ALP, ALB and TBIL,repectively.Results: There was obvious increase in the serum concentration of ALT in group A and group B within normal reference range ( P <0 01). The serum concentration of ALB was obviously decreased 2 h after induction in all four groups ( P <0 05 ), but returned to normal 3 d after operation . There were no any significant difference among four groups, either ( P >0 05). There were no significant differences in other parameters. Conclusion:Both enflurane and sevoflurane have slight potential liver toxicity ;Propofol in clinical dose can obviously prevent the increase of aminotransferase activity and protect the liver from toxicity caused by enflurane and sevoflurane; combined propofol enflurane or propofol sevoflurane anaesthesia is a ideal general anaesthetic method.
