Objective To investigate the correlation between the abnormalities of transcranial magnet stimulated motor evoked potentials (MEP) and clinical conditions of patients with cerebral infarction.Methods 30 cases of cerebral infarction patients and 30 age and sex matched healthy controls were examined for transcranial magnet stimulated MEP and complex muscle action potentials(CMAP) were recorded from abductor digiti minimi(ADM) and biceps brachii in bilateral upper extremities.Results The results showed that the cortical latency in paralyzed side was lengthened from (20.8±0.8)ms to (23.0±0.9)ms (P<0.01).Central motor conduction time (CMCT) in lesioned side was delayed from (7.9±0.7)ms to (10.2±0.8)ms (P< 0.01), while duration was widened from (16.9±1.1)ms to (20.7±1.3)ms (P< 0.01) and amplitude decreased from (0.9±0.6)mV to (0.4±0.2)mV (P<0.01) in the paralyzed side.Degree and site of MEP abnormalities is well correlated with clinical paralyzed sites as well as degree of muscle power.In group 1 with muscle power over grade 4,evoked potentials could be induced under no muscle contraction condition in all 11 cases,while in group 3 with muscle power less the grade 2,no potentials could be elicited in all 9 cases even under muscle contraction condition.Conclusions Transcranial magnetic MEP can be used to understand the functions of corticospinal tracts in patients with cerebral infarction and to judge the clinical severity of such patients.

目的 探讨经颅磁刺激运动诱发电位 (MEP)异常与缺血性脑卒中患者临床病情之间的相关性及临床意义。方法 对 30例缺血性脑卒中患者分别进行经颅磁刺激 ,双侧外展小指肌 (ADM)记录运动诱发电位。结果 临床肌力 (0~ 5级 ) 4级以上者 11例患者在静止状态下可引出MEP波 ,10例肌力 3级者在轻收缩状态下可引出MEP波 ,肌力 0~ 2级的 9例患者不能引出MEP波。脑梗死组病侧中枢传导时间 (CMCT)比健侧明显延长由 (7.9± 0 .7)ms延长至 (10 .2± 0 .8)ms。通过对MEP波的潜伏期、波幅、中枢传导时间以及对照组 30例的MEP波进行统计学分析 ,发现MEP异常与临床运动功能障碍密切相关 ,能反映卒中患者病情的轻重。结论 经颅磁刺激运动诱发电位能较好地反应缺血性脑卒中患者的病情 ,可用于临床判断病情。

目的 研究睫状神经营养因子 (CNTF)对周围神经损伤后再生的影响。方法 用硅胶管桥接大鼠双侧坐骨神经缺损 ,形成神经再生室。左侧室内注入基因重组人CNTF ,右侧注入生理盐水(NS)。术后 14、30、6 0和 90天取材 ,行大体、光镜、电镜观察。术后 90天行轴突图像分析、电生理检测及霍乱毒素—辣根过氧化物酶 (CB HRP)逆行追踪。结果 与对照侧相比 ,实验侧再生有髓神经纤维数目较多、轴突较粗且髓鞘较厚 ,复合肌肉动作电位 (CMAP)的潜伏期较短、神经传导速度较快且波幅较高。实验侧CB HRP标记的脊髓前角运动细胞数明显多于对照侧。结论 外源性hCNTF有明显促进周围神经再生作用

OBJECTIVETo study the expression and mechanism of basic fibroblast grouth factor(bFGF) and transforming grouth factor(TGF-β) on repairing the long bone defect with transplanting complex muscle flap, bone marrow and granule of cancellous bone.METHODSThe 20mm of bone defect models were made in each side of radius, and muscle flap, bone marrow and granule of cancellous bone were transplanted in experimental group, and muscular flaps were transplanted in control group.The hitological staining and immunohistochemistry staning of bFGF and TGF-β were observed in the different session.RESULTSThe chondrocyte, osteoblast and osteocyte were highly expressed bFGF and TGF-β in experimental group after third days second, 4th, 6th, 8th weeks of operation. In control group, bFGF and TGF-β were expressed in small number of chondrocyte after 4 weeks of operation, and that were expressed in osteocyte of the new formed bone of cut ends after 6th and 8th weeks of operation, but the number of expressing cells are more than that of experimental group significantly.CONCLUSIONThe bFGF and TGF-β in the part of bone defect were highly expressed and released in combined transplantation of muscular flap, bone marrow and granule of cancellous bone in the early stage, and the bone defect can be repaired earliy and better by that.

[目的 ]探讨肌肉瓣与骨髓加松质骨颗粒联合移植修复长段骨缺损中碱性成纤维细胞生长因子 (bFGF)及转化生长因子 (TGF β)的表达及修复机理 .[方法 ]在健康中国家兔双侧桡骨中段制作 2 0mm的长骨缺损模型 ,实验组行肌肉瓣与骨髓加松质骨颗粒联合移植 ,对照组单纯植入肌肉瓣 .不同时期进行各组家兔的组织学观察以及bFGF与TGF β多克隆抗体的免疫组织化学染色 .[结果 ]行移植术后 2 ,4 ,6 ,8周时实验组家兔软骨细胞、成骨细胞及骨细胞的bFGF及TGF β呈强阳性染色 .对照组家兔在移植修复 4周时肌肉组织间形成的软骨细胞中有少量软骨细胞呈阳性染色 ,6周及 8周骨缺损两端新生骨痂中部分骨细胞的bFGF及TGF β呈阳性染色 ,但阳性染色细胞数与实验组相比有显著性差异 .[结论 ]肌肉瓣与骨髓加松质骨颗粒联合移植早期在骨缺损局部中bFGF与TGF β的表达与分泌旺盛 ,促进诱导成骨 ,较快地修复长段骨的缺损 .
