Objective To investigate the impact of one versus two distal locking bolts on the mechanical properties of tibial interlocking intramedullary nails. Methods Twenty 9-mm titanium alloy locking nails were divided into two equal groups in which the nails were fixed with only one and two distal locking bolts, respectively. Each group was further divided into two sub-groups for compression and torsion tests separately using a universal material-testing machine. Results In the compression tests, the average maximum strength of double bolts was greater than that of the single bolt (P<0.05), but the number of the bolts did not significantly affect the results of the torsion tests in terms of the either the maximum torsion moment or angle (P>0.05). Conclusion One distal bolt is sufficient for fixing stable fractures and double bolts are recommended for management of serious fractures. The model we used is convenient and economic for examining the biomechanics of the tibial nails, especially for comparative purposes.

目的研究髓內钉远端锁钉数目对胫骨加锁髓内钉整体力学性能是否有影响?椒ㄊ笛楣惭∮?0根国产钛合金制造的9mm胫骨加锁髓内钉。将所有髓内钉随机分成两组,即远端安装单枚锁钉的单锁钉组,和远端安装2枚锁钉的双锁钉组。每组中各随机选用5根分别在万能力学实验机上进行压缩和扭转力学测试。测试所用的模型为根据国内外实验结果自行研究的不锈钢模型。结果在压缩实验中,单钉组平均最大压缩载荷为1850N,双钉组的平均最大载荷为2150N,双钉组抗压缩能力显著高于单钉组(P<0.05)。在扭转实验中,单钉组平均最大扭矩、扭角分别为55.54N·m和57.36°,双钉组分别为55.78N·m和58.28°,两组差异无统计学意义(P>0.05)。结论胫骨髓內钉远端用单枚锁钉时其整体力学性质能满足临床要求?杂诮涎现氐墓钦劾嘈突故峭萍霭沧傲矫对抖怂ぁ1臼笛椴捎玫牧ρР饬磕P头奖闶涤们揖靡椎?尤其适于髓内钉的力学对比研究。

The third nail endured much strain than the fourth nail on each condition. Conclusion The distal nails endured much strain, especially the third nail. Interlocking nails under the crack, especially very near it, tend to be broken, so that two nails should be applied to the distal fracture and their position should be apart from the crack as far as possible.

目的对胫骨交锁髓内钉远端不同锁钉方式进行生物力学测试,为临床交锁髓内钉治疗胫骨骨折提供理论指导。方法以成人新鲜胫骨标本为试验对象,每具标本先后模拟骨折愈合、稳定骨折及不稳定骨折3种状态,远端采用1枚或2枚锁钉固定,MTS轴向加载0~1000N,测试锁钉应力应变变化。结果随着骨骼稳定性的变化,各锁钉应力应变均发生显著性变化,在远端2枚锁钉中,靠近侧的锁钉应力应变更大。远端改用1枚锁钉固定后,远端锁钉承受的应力应变进一步显著增加,且以离骨折线较近的1枚锁钉固定应力应变更大。结论骨折线远端的锁钉应力应变大,易发生断裂及退钉,尤其是靠近骨折线者;应尽量以2枚锁钉固定远端并尽量远离骨折线。

Objective To investigate the operation method and effect of floating knee. Methods 33 cases of floating knee were recruited and treated with variant methods according to fracture site, type, and soft tissue injury. 6 cases were treated with femoral compression plate and tibial external fixation, 10 with femoral interlocking intramedullary nail and tibial external fixation, 15 with femoral interlocking intramedullary nail and tibial interlocking intramedullary nail, and 2 with femoral interlocking intramedullary nail and tibial condylar plate. Results 30 cases were followed up for 1~5 years, 2.75 on average. The excellent and good rate was 86.7%. Conclusions Early operation is beneficial for the treatment of multiple injuries and joint function recovery. The operation method should be selected according to fracture site, type, and soft tissue injury.

目的探讨手术治疗漂浮膝损伤的方法和疗效。方法对33例漂浮膝损伤的患者,根据骨折部位、骨折类型及软组织损伤情况,使用股骨加压钢板-胫骨外固定架固定6例,股骨交锁髓内钉-胫骨外固定架固定10例,股骨交锁髓内钉-胫骨交锁髓内钉固定15例,股骨交锁髓内钉-胫骨髁钢板内固定2例。结果3例失访,30例经1~5年随访,平均2年9个月,优良率86.7%。结论早期手术治疗有利于严重多发伤的治疗和关节功能的恢复,手术方法应根据骨折部位、类型及软组织损伤情况,采用合适的方法。
