Objective:To investigate the feasibility and efficiency of autologous intracoronary mononuclear bone marrow cell transplantation for treatment of dog myocardial infarction. Methods: Ten male dogs underwent permanent left anterior descending coronary arteries occlusion by balloon to establish myocardial infarction(MI) model. About 1 week after extensive MI, dogs were divided into transplantation group (n=5) and control group(n=5). Echocardiography study was performed before and 60 d after implantation to assess left ventricular remodeling and function. Dogs were sacrificed and specimens were harvested 60 d post-implantation;histologic sections were examined under a fluorescent microscopy. Results: The left ventricular end-diastolic volume(LVEDV) and left ventricular end-systolic volume(LVESV) decreased in transplantation group, while EF and FS increased significantly(P<0.05) and SV insignificantly. The LVEDV and LVESV in control group increased obviously, while FS decreased significantly(P<0.05) and SV and EF insignificantly. The LVESV in transplantation group was significantly lower than that of control group(P<0.05) while LVEDV insignificantly; EF and FS in transplantation group was significantly higher than that of control group(P<0.05) while SV insignificantly. The LV diastolic and systolic enternal diameter and the length of infarction area decreased after transplantation and the thickness of infarction area increased(P<0.05), while the thickness in control group decreased. In transplantation group the cardiac anterior wall was positively stained under fluorescent microscope with green fluorescent myofilaments and myotubes, while the fluorescence in inferior wall of transplantation group and anterior wall of control group was weaker and with no fluorescent myofilaments and myotubes.Conclusion: Autologous mononuclear bone marrow cells can be implanted and survive in the infarcted myocardium, which can attenuate infarct thinning, myocardial remodeling and dysfunction, and may provide a new approach for myocardial repair.

Objective:To investigate the possibility, safety, and effects of repairing infarcted myocardium and angiogenesis with autologous mononuclear bone marrow cells (MBMC) transplantation by echocardiography-guided percutaneous trans-extracardial injection in dogs with severe ischemic heart failure. Methods: Permanent left anterior descendant coronary arteries occlusion was performed in 20 male dogs by balloon to establish ischemic heart failure and extensive myocardial infarction(MI) models. Ten days after MI, echocardiography was performed and EF <40% was considered as severe ischemic heart failure models. Fourteen dogs were divided into 2 groups: cell transplantation group (n=6) and control group(n=8). One day before cell transplantation, MBMC were harvested, isolated, washed, labelled with 4',6-dimidino-2-phenylindone(DAPI). Echocardiography guiding was used to identify infarcted myocardium area percutaneous trans-extracardial injection with 23G ultrasonically guided needle. Control group was injected with saline only (10 injections of 0.2 ml each). Echocardiography study was performed before and 3 months after cells graft. Results: Fluorescent labelled MBMC with green color were found in cell transplantation group 3 months later, but not in control group. Light microscopy revealed the presences of engrafted MBMC and the presence of nascent intercalated disks connecting the engrafted cell and the host myocardium. Many new blood vessels were formed in the infarct-bed. There was a significant improvement in cardiac function. Compared with those before cells transplantation, the strove volume increased by 16.7 ml, ejection fraction increased by 14.7%, and left ventricular diameter decreased by 6%.Conclusion:It is feasible, safe, and effective to use echocardiography-guided percuteneous trans-extracardial injection of MBMC in prepairing infarted myocardium, which might provide a new approach for treatment of severe ischemic heart failure patients who are not suitable for percutaneous coronary intervention and bypass surgery.

Objective To investigate the possibility、 efficacy、 safety of the repair of infarcted myocardium and improvement of cardiac function by intracoronary autologous mononuclear bone marrow cell (MBMC) transplantation in patients with AMI. Methods Thirty-eight patients with AMI were enrolled in this prospective study and divided into PCI+cell transplantation group and PCI only group. Baseline and follow up evaluations included complete clinical and laboratory evaluations, 2D echocardiogram, positron emission tomography (PET), 48 ECG monitoring. MBMC were harvested, isolated, washed, and resuspended in saline for infusion. 21 patients were transplanted with autologous MBMC via a balloon catheter placed into the infarct-related artery during balloon dilatation by highpressure infusion 14.6 days after AMI, which was performed 6-8 times for 2 minutes each and 7 patients were transplanted via non-selective infarct-related arteries by highpressure infusion. Results Surgery was safe. 3 patients had limited premature ventricular contractions during cell infusion for several seconds. 3 patients felt cold after 15-30 minutes infusing cell. After 3 months of fellow-up, there were viable myocardium in the infarct defected regions in 13 cases after MBMCs transplantation determined by PET, which occupied [(40.08±8.82%), P<0.01] of infarct defected region in 13 patients with AMI. Cardiac function in patients with AMI were improved: Ejection fraction (EF) increased 6.83% (P<0.01); Stroke volume increased 18.63% (P<0.05). But in control group cardiac function was not improved. Conclusion Present study demonstrates that intracoronary transplantation of autologus MBMC is safe and effective. The marked therapeutic effect may be attributed to MBMC-associated myocardial regeneration and neovascularization as well as the action of cytokines and growth factors.
