应用自制改良式Ilizarov外固定架对30只山羊施行左胫骨骨干延长术。术后第7,14,21,28,35,49和63天分别各处死4只山羊。标本行光镜、电镜观察,发现张力-应力下骨延长骨发生符合膜内成骨,矿化沿定向胶原束逐渐形成板层骨,5~6周后成骨细胞出现,同时骨小梁直径增粗。愈合早期大多数成纤维细胞直接演变为骨细胞,在骨细胞的四周同时形成骨隐窝,使其发生可以不完全依赖于骨外膜及骨内膜的成骨细胞或骨生成细胞,能更早地在延长上、下端形成骨组织。说明成纤维细胞不仅产生胶原,而且在Ilizarov再生骨发生中起重要的作用。

The aim of this study was to promote an effective approach, which was surgicallyassisted buccal mandibular corticotomy in the lockbite side to rapidly expand mandibular arch to correct unilateral posterior lockbite using the principle of the distraction osteogenesis. The typical subject was a 16yearold patient with unilateral posterior lockbite. The orthodontic treatment had been done before the surgery. After the surgery, reciprocal elastic traction between upper and lower arch were used immediately...

利用牵张成骨(distractionosteogenesis)原理,设计了一种新的、可供临床应用的矫正单侧全部后牙锁的正畸-外科联合治疗方法。按设计在下颌第一双尖牙近中、最后磨牙远中及该骨段根尖下3mm作骨皮质和骨松质切开术;术毕即用术前设计制备的口内牵张器矫治牙颌畸形。对2例单侧全部后牙锁的治疗结果表明,锁畸形完全矫正,咀嚼功能恢复正常,疗程短而效果稳定,是一种行之有效的方法。文中还对该方法的适应证、治疗过程和方法等进行了讨论

目的总结小儿肢体延长的临床经验,探讨肢体延长中出现的并发症及其对策。方法采用Ilizrov外固定延长架对14例小儿下肢短缩畸形进行15侧肢体延长。其中股骨下端截骨延长3例,胫骨上端截骨延长9例(10侧肢体),胫骨下端截骨延长2例。结果随访1~6年,平均2年3个月。延长幅度3.0~10.0cm,平均6.5cm。延长率达原骨长度的12.5%~45.5%,平均22.9%。骨愈合时间:股骨延长平均280天,胫骨延长平均215天。骨愈合指数平均41.5。结论应用Ilizarov外固定延长架可以较大幅度延长肢体,但应充分重视延长中各种并发症的预防和处理
