A semi-nested PCR was performed to amplify the CDR Ⅲ fragments of IgH gene from 70 lymph node specimens,Clonal rearrangement band of the CDR Ⅲ was delected in 40(87%)of 46 cases with Bcell lymphomas.Minimal residual disease was detected with case specific probes.Of 3 complete remission cases in stage Ⅳ at diagnosis,2 showed positive signal and relapsed within one year.The other one was negative and still in CR.

A total of l2 patients gave positive results, they include 9 of the 15 patients with follicular lymphoma and 3 of the 30 patients with diffuse B-cell lymphoma. The results were negative in 5 patients with reactive hyperplasia. The breakpoint was located within the major breakpolnt region in 11 of the 12 patients, and the remaining patient had bcl-2 translocation in the rninor cluster region. The results of the study show that bcl-2/JH fusion gene in non-Hodgkin's lym phomas is an important molecular biological marker and has a significant role in differentlating benign or malignant hyperplasia, determination of clonality, predicting prognosis and in the discovery of minor residual disease.

为研究我国非何杰金淋巴瘤(NHL)患者bcl-2基因重排的发生情况,以及其可能的临床应用价值,应用半套式聚合酶链反应(semi-nestedPCR)检测45例非何杰金淋巴瘤bcl-2/JH融合基因,发现12例阳性,其中15例滤泡性淋巴瘤中有9例阳性(60%);30例弥漫性淋巴瘤中3例阳性(10%),5例反应性淋巴组织增生者阴性。并发现bcl-2基因断裂点绝大部分位于主要断裂区(11/1291.7%),少数位于次要断裂区(1/12,8.3%)。结果提示,形成bcl-2/JH融基因是B细胞非何杰金淋巴瘤发生的重要病因,检测bcl-2/JH融合基因对鉴别淋巴组织的良恶性增生、明确克隆来源,提示预后及检测微小残留病灶均有重要意义。

In this study, the single-ture primers and semi-nested primers were used in the PCR procedure for detecting IgH and TCR gene rearrangements in the same DNA sample from bone marrow of 25 children suffering from acute lymphoblastic leukemia (ALL), and the positive rates were compared. In this study, the single-ture primers and semi-nested primers were used in the PCR procedure for detecting IgH and TCR gene rearrangements in the same DNA sample from bone marrow of 25 children suffering from acute lymphoblastic leukemia (ALL), and the positive rates were compared. Results, the positive detectable rate of semi-nested PCR was 80%,that of single-ture PCR was 52%.The former increased by 28% as com pared with the latter. Particularly, for the detecting of ALL-MRD group, the positive detectable rate of the semi-nested PCR remarkably improved. The results showed that semi-nested PCR is more sensitive and specific in detecting IgH and TCR gene rearrangements of ALL-MRD. But for distinguishing T、B cellular lineage of monoclonal cells,TCR βgene rearrangement was more significant.

根据IgH、TCR克隆性基因重排原理,用半重叠PCR和单轮PCR技术分别对同一份ALL患儿的骨髓标本进行检测(共25例),并对两法的检出率进行了比较。结果:半重叠PCR阳性捡出率为80%,单轮PCR阳性检出率为52%,前者较后者提高了28%,尤其对ALL-MRD组的检测,半重叠PCR的阳性检出率有显著提高。结果表明,半重叠PCR双轮扩增IgH、TCRγ基因重排对于MRD的检测起到增敏效果,可减少非特异性产物的生成,大大提高了MRD的阳性检出率,并与单轮PCR同样具有简便、快速以及用DNA量少,对DNA模板质量要求低,无放射性污染等优点。对区别肿瘤细胞的T、B细胞源性TCRβ基因重排尤其有价值。
