Phosphata'se (ALP) and creatine kinase (CK) were not helpful for diagnosing ischemic hepatitis. It is suggested that GDH may be the most sensitive and specific serum marker in ischemic hepatitis.

目的探讨谷氨酸脱氢酶(GDH)、胆碱酯酶(CHE)在高血清丙氨酸转氨酶(ALT)患者鉴别诊断中的应用价值。方法收集103名健康者和108例高ALT患者的血清标本,在BayerADVIA1650型全自动生化分析仪上同时测定GDH、CHE的活性。结果在急性病毒性肝炎、急性缺血性肝炎、急性中毒性肝坏死时GDH均明显升高,与正常对照组差异有显著性(P<0.01),其中以急性缺血性肝炎升高的幅度最大,其和急性中毒性肝坏死的阳性率均可达100%。CHE只有在急性中毒性肝坏死时才明显下降(P<0.01)。结论联合检测GDH、CHE活性将有助于鉴别诊断高血清ALT肝病患者,对指导临床治疗具有重要意义。

Objective:To anglyse the clinical and laboratory features of ischemic hepatitis.Methods:Two male patients(above 70 years old) admitted with left heart failure and respiratory tract infection were reported and with literature review.For both the two patients,serum aminotransferase rose rapidly and dramatically,while dropped within a few days.The change was not parallel to the severity of disease and could not be explained by viral hepatitis,toxic or drug-induced liver damage.Results:The diagnosis of ischemic hepatitis was established for two patients from clinical and laboratory data.Conclusions:The ischemic hepatitis exhibits particular clinical and laboratory features.It is necessary for the physicians to distinguish ischemic hepatitis from liver damage caused by other factors.Liver enzyme levels should be monitored in elderly patients with multiple underlying systemic diseases and conditions,especially those with left heart failure.Special care should be taken to prevent ischemic hepatitis.

目的:分析缺血性肝炎临床及实验室特征。方法:文献复习并报告本院收治的2例老年男性患者,年龄均>70岁,在左心衰及呼吸道感染病程中突然出现转氨酶急剧增高,并在短期内快速下降,与疾病病情不平行,不能用病毒性肝炎、中毒或药物引起的肝损害解释。结果:结合临床及实验室特点,符合缺血性肝炎诊断。结论:缺血性肝炎具有独特的临床和实验室特征,临床医生应当注意鉴别缺血性肝炎与其他原因引起的肝脏损害。对患有多种全身性疾病(尤其是左心衰)的老年患者,需要定期监测血清转氨酶水平,预防缺血性肝炎的发生。
