Objective:To examine the efficacy of allogeneic cortical bone as an alternative substrate for cervical reconstruction after anterior decompression.Method:Fifty-seven patients underwent anterior cervical decompression implanted with cortical bone allograft in stead of iliac crest autograft.The clinical results,cervical curvature and radiographic fusion were investigated during mean 23.1 months’ follow-up.Results:Clinical myeolopathy score was increased by 5.5 points.The cervical lordosis was well preserved.Implant complications occurred in 3 cases within 5 months postoperatively,two cases had revisional operations.Revasculation and interface fusion were found at 5 to 6 months postoperatively by radiography and bone scan.For all the patients followed up for more than one year,bony fusion were achieved.Conclusion:Allogeneic cortical bone can be used as an efficient alternative substrate for anterior cervical reconstruction.

目的 :观察同种异体皮质骨用于颈椎前路减压术后重建的临床疗效。方法 :自 2 0 0 1年 1月~ 2 0 0 3年 1月 ,应用异体皮质骨替代自体髂骨移植治疗 5 7例颈椎疾病患者 ,平均随访 2 3 1个月 ,对临床疗效、颈椎曲度及植骨愈合情况进行随访观察。结果 :脊髓功能评分平均增加 5 5分 ,手术疗效优良率为 84 2 %;颈椎前凸保持良好 ;术后5个月内出现植骨并发症 3例 (占 5 2 %) ,其中 2例行二次手术 ;术后 5~ 6个月拍片及骨扫描示植骨融合 ,随访 1年以上患者骨性融合率达 10 0 %(19/ 19)。结论 :同种异体皮质骨可替代自体髂骨植骨用于颈椎前路减压术后重建。

Objective:To discuss the effects of histocompatibility of cortical bone plates allografts treated by deep-freezing,ethylene oxide sterilization and gamma rays radiation.Methods:Forty-eight goats underwent allogeneic cortical bone plates transplantation treated by deep-freezing,deep-freezing plus 48℃ ethylene oxide,and deep-freezing plus irradiation with 25kGy gamma rays as well as fresh cortical bone plates.Groups of goats were sacrificed and specimens were procured at 3-,6-,12-,and 24-week after surgery for examination CD4 +, CD8 + in blood, infiltration of lymphocytes and macrophages surrounding soft tissue of bone plates in order to evaluation biocompatibility of the implant.Results:CD4 + in blood was increased,and there were severe infiltration of lymphocytes and plasmocytes surrounding soft tissue of bone plates after operation in the fresh bone plates group. The step-up of T-lymphocytes and infiltration of inflammation cells achieved the peak at 6-week after operation in the fresh bone plates group.T-lymphocytes of groups in blood was normal and there was few inflammation cells in the soft tissue surrounding the bone plates in the deep-freezing,deep-freezing +ethylene oxide and deep-freezing +irradiation with 25kGy gamma rays groups.Conclusion:There was strong antigen and severe immune response in allogeneic cortical bone plates transplantation.The biocompatibility of allograft strut treated by deep-freezing,deep-freezing plus 48℃ ethylene oxide,and deep-freezing plus irradiation with 25kGy gamma rays was very well tolerated and the methods uesd above was very useful to allograft cortical bone plates in elimination antigen and sterilization.

目的 :探讨深低温冷冻、环氧乙烷和γ射线辐照对同种异体皮质骨板移植组织相容性的影响。方法 :分别移植经过 - 70℃深低温冷冻 4周、 - 70℃深低温冷冻 4周 +48℃环氧乙烷灭菌、 - 70℃深低温冷冻 4周 +2 5kGyγ射线辐照处理的山羊同种异体皮质骨板 ,对照组移植异体山羊新鲜皮质骨板。检测山羊外周血CD4 + 、CD8+ 以及移植骨板周围软组织炎性细胞浸润。结果 :新鲜骨板移植组CD4 + 明显升高 ,移植骨板周围软组织大量淋巴细胞、浆细胞等炎性细胞浸润 ,CD4 + 升高以及炎性细胞浸润在术后 6周达到顶峰。除深低温组术后 6周CD4 + 升高外 ,深低温冷冻、深冻 +环氧乙烷及深冻 +γ射线辐照组T淋巴细胞亚群无明显改变 ,移植骨板周围软组织少量淋巴细胞等炎性细胞浸润。结论 :新鲜同种异体皮质骨板移植有较强的抗原性 ,诱发宿主较强烈免疫排斥反应 ;深低温冷冻、环氧乙烷和γ射线辐照处理皮质骨板移植宿主不发生免疫排斥反应 ,组织相容性较好。

Groups of goats were sacrificed and specimens were procured at 3 ,6 ,12 ,and 24 week after surgery for examination CD4 +, CD8 + in blood.Seven patients with periprosthetic fractures of hip underwent cortical bone plates allografts by deep freezing at -70℃ after being treated of 48℃ ethylene oxide.To detect T lymphocytes,antibody,complement and circulation immunity complex (CIC) in blood.Result:Animal experiment showed that CD4 + in blood increased, and the step up of T lymphocytes achieved the peak at 6 week post operation in the fresh bone plates group.T lymphocytes of groups in blood were normal in deep freezing, deep freezing+ethylene oxide and deep freezing+radiation with 25kGy gamma rays groups in animal. T lymphocytes, antibody, and circulation immunity complex (CIC) in blood were normal post operation in patients.Conclusion:There is strong antigen and immune response in allogeneic fresh cortical bone plates transplantation, but there are no immune response in animal and patients who received allograft strut treated by deep freezing, deep freezing plus 48℃ ethylene oxide, and deep freezing plus radiated with 25kGy gamma rays.

目的: 探讨实验动物和患者接受不同方法处理的同种异体皮质骨板移植后宿主免疫学反应。方法: 48只山羊接受分别经过 -70℃深低温冷冻 4周、 -70℃深低温冷冻 4周 +48℃环氧乙烷灭菌、-70℃深低温冷冻 4周+25kGyγ射线辐照处理的同种异体皮质骨板移植,对照组移植异体山羊新鲜皮质骨板。7例髋关节置换术后股骨假体周围骨折患者, 接受深低温冷冻和环氧乙烷处理同种异体皮质骨板移植重建股骨骨折。检测实验动物外周血CD4+、CD8+以及患者外周血T淋巴细胞亚群、抗体、补体和循环免疫复合物。结果: 新鲜骨板移植组CD4+明显升高, 术后 6周达到顶峰。除深低温组术后 6周CD4+升高外, 深低温冷冻、深低温冷冻 +环氧乙烷及深低温冷冻 +γ射线辐照组T淋巴细胞亚群无明显改变。患者T淋巴细胞亚群、抗体和免疫复合物正常, 切口无感染。结论: 新鲜同种异体皮质骨板具有抗原性, 移植后诱发宿主免疫排斥反应; 深低温冷冻、深冻+环氧乙烷和深冻 +γ射线辐照处理同种异体皮质骨板移植受体无明显免疫排斥反应。
