Objective To explore the mechanisms of the hyperalgesia in the spinal cord and the effect of the morphine on the c-fos expression in spinal dorsal horn by investigating the changes of pain threshold elicited by touch-stimulus after the chronic constriction injury(CCI)of the sciatic nerve in rat.Methods Thirty female Sprague-Dawley rats were randomly divided into sham-operation group,CCI group and morphine group.Mechanic pain threshold and thermal pain threshold were assessed on the second day before operation and on the 4,7,14 th postoperative day in three groups.The rats were given innoxious touch-stimulus for 3 hours before killed.The spinal cord was obtained after death.Immunohistochemistry was employed to investigate the expression of the c-fos.Results The hyperalgesia appeared on the 4 th day after CCI-operation of the sciatic nerve and reached its maximal level on the 14 th day after CCI operation,the expression of c-fos elicited by touch-stimulus increased significantly and it mainly distributed at middle of superficial layer in laminal Ⅰ-Ⅱ in spinal dorsal horn in CCI group by touch-stimulus.There was an increase in laminal Ⅴ-Ⅹ.The hyperalgesia was not attenuated after 2 mg/kg morphine injected intraperitoneally after chronic constriction injury of the sciatic nerve in rats.The expression of c-fos did not significantly decrease in morphine group(P<0.05)and there had no significant difference in c-fos expression between the CCI and morphine group.Conclusion Hyperalgesia and the expression of the c-fos were not inhibited effectively following 2 mg/kg morphine injected intraperitoneally after chronic constriction injury of sciatic nerve in rats.The results may be related to the administration pathway and the doses of morphine used in this study was too low to disrupt the hyperalgesia,or mechanism may relate to excitability elevation of the secondary sensory neurons in the spinal dorsal horn.

目的 观察慢性坐骨神经收缩损伤 (CCI)大鼠触摸刺激后机械痛阈和热痛阈的变化 ,以研究吗啡 (2mg/kg)腹腔注射后对非伤害性刺激后脊髓后角c fos表达的影响 ,探讨脊髓痛觉调控机制。方法 选择雌性SD大鼠 30只 ,随机分为假手术组、CCI组和吗啡组 ,分别于术前 2d、术后 4、7、14d给予机械痛阈和热痛阈测试。大鼠处死前 3h给于非伤害性触摸刺激 ,处死后取脊髓切片 ,免疫组织化学方法检测c fos表达。结果 CCI组和吗啡组大鼠坐骨神经结扎 4d后均呈现不同程度的痛觉过敏 ,以术后 14d为甚 ,触摸刺激导致的脊髓c fos的表达明显增加 ,主要集中分布于脊髓背角表层Ⅰ~Ⅱ层的中间内侧部 ,Ⅴ~Ⅹ层也有一定程度增多。 2mg/kg腹腔注射不能有效的阻断慢性缩神经损伤大鼠痛觉过敏。吗啡组与CCI组相比c fos的表达无明显减少 ,两组相比差异无显著性意义 (P >0 0 5 )。结论  2mg/kg吗啡腹腔注射不能有效抑制非伤害性刺激诱发痛觉过敏以及脊髓背角c fos的表达 ,可能与吗啡用药途径和用量过小有关 ,也可能与脊髓背角二级感觉神经元兴奋性增高有关

Objective To study the changes of synaptophysin in the rat spinal cord after chronic constriction injury of sciatic nerve and the influence of ketamine intervention.Methods Twenty-four female SD rats weighing 180-250 mg were divided randomly into 4 groups (n=6 in each group).Group I:The right sciatic nerve chronic constriction injury (CCI) model was established.Group Ⅱ:Sham-operative control.Group Ⅲ:Ketamine 5 mg/kg was administered intraperitoneally on day 4 after CCI,twice everyday and continuously for 10 days.Group Ⅳ:The same procedure as Group Ⅲ was taken and the dosage of ketamin was changed to 10 mg/kg.Thermal nociceptive and mechanical withdrawal thresholds were measured before the constriction injury and on the day 4,7 and 14 post-chronic constriction injury.The changes of synaptophysin in the spinal cord was detected quantitatively by using immuneohistochemistry and computerized image analysis.Results Pain threshold in groups I,Ⅲ,Ⅳ was reduced significantly on the day 4 after sciatic nerve ligation as compared with that in group Ⅱ (P< 0.05) and the established heat hyperalgesia and mechanical allodynia in groups I,Ⅲ was maintained for 10 days.Thermal pain threshold and mechanic pain threshold in group Ⅳ were increased on the day 14 as compared with those in group I (P< 0.05). The density of synaptophysin immunoreactivity in the superficial dorsal horn as compared with that in control group was remarkably increased by 58% on the 14th day after CCI (P<0.01).The synaptophysin-positivity in group Ⅳ was significantly reduced by 25% as compared with that in CCI model rats (P< 0.05). Conclusion There is an increase of synaptophysin immunoreactivity in the superficial layers of the dorsal horn during the development of neuropathic pain and ketamine can restrain this change.

目的探讨坐骨神经慢性压迫性损伤大鼠脊髓突触体素(p38)变化及氯胺酮干预的影响。方法将24只SD雌性大鼠随机分为4组,I组:制备坐骨神经慢性压迫性损伤(CCI)模型;Ⅱ组:假手术对照组;Ⅲ组:CCI手术后第4天开始每天腹腔注射氯胺酮5mg/kg体重2次,持续10d;Ⅳ组:方法同Ⅲ组,但氯胺酮剂量为10mg/kg体重。定期观察大鼠手术侧辐射热痛阈和机械痛阈的变化。用免疫组织化学技术和计算机图像分析系统检测手术侧脊髓背角浅层突触体素表达的变化。结果与对照组相比,其他3组均在第4天痛阈明显降低(P<0.05),此后I组、Ⅲ组痛阈一直稳定在较低水平即痛敏状态,而Ⅳ组在第14天热痛阈值和机械痛阈值与I组相比有明显升高(P<0.05)。I组手术侧脊髓突触体素免疫阳性产物平均吸光度值与Ⅱ组相比增加了58%(P<0.01),Ⅳ组与I组相比降低了25%(P<0.05)。结论神经病理痛发生后突触体素在脊髓背角浅层的表达明显升高,氯胺酮对其有抑制作用。

Objective To compare the establishment and afficacy of three neuropathic pain models:chronic constriction injury of sciatic nerve(CCI), spinal nerve ligation (SNL) and spared nerve injury(SNI). Methods Nineteen female SD rats (weighted from 150 to 200 g ) were randomized into four groups(group C,group CCI, group SNL, group SNI). The sciatic nerve of the left hind leg was ~exposed without any injury in group C, then the incision was sutured immediately. Group CCI was prepared according to the method described by Gary,group SNL according to Sun, and group SNI according to Isabelle. During the development of allodynia and ~hyperalgesia, touch stimulator and radiant heat stimulator were used to measure the response of operative hind paw to mechanical and radiant stimulation. Results During experiment, any sign of ~mechanical or heat allodynia did not occur in group C. But the surgical procedure in group CCI, SNL and SNI produced an apparent and stable hyperalgesia to noxious heat and mechanical stimulation since the 7th day after operation. On the 7th day after operation, the 50% paw withdraw thresholds in group C,CCI, SNL and SNI were 14.13±1.49,2.55±0.47,1.52±0.58 and 0.79±0.36 respectively,with an quantity order of group C>CCI>SNL>SNI(P<0.05). The difference in scores responded more rapidly to radiant heat stimulation was lower in group CCI than that in group C. But there was not any difference between group SNI and group C in responce to radiant heat stimulation.Conclusion Among three pain models, SNI was not only model easiely established, but also the most sensitive to ~mechanical stimulation(but not radiant heat stimulation).
