It was reported that coagulation and fibrinolysis functions were dete- rmined by chromogenic peptide substrate,S-2238 and S-2251,at the end of operation in 35 cases,of which the thoracic operations were 15 cases,abdominal operation 6,orthopedic operation 9 and cranial operation 5.Age ranged from 14 to 73 years,with a mean ageof 40.7±15.3.The shortest operative time was 1.5 hours,the longest 8 hours,with a mean time of 4±1.5 hours.The activities of AT-Ⅲ,PLG and APL were measured by chro- mogenic peptide substrate assay methods,The results of all groups were lower than nor- mal levels and the differences were of statistical significance(p<0.01).However,the act- ivity was still above 60%.The operative time had no influence on the activity.This lowered activity of coagulation and fibrinolysis was a protective reaction of organism immediately after surgical injury,since there were no clinical symptoms of DIC.It was related to the degree of trauma,but not to the operative time.It is concluded that to mo- nitor coagulation and fibrinolysis functions in patients receiving major operations is valuable to some extent in predicting the prognosis and postoperative complicatios.

本文报道35例大手术病人,术终利用显色肽基质S-2238和S-2251测定凝血和纤溶功能的变化。35例病人,包括胸部手术15例、腹部手术6例、骨科手术9例和颅脑手术5例。年龄范围14~73岁,平均为40.7±15.3岁。手术时间最短为1.5小时,最长为8小时,平均为4±1.5小时。按照显色肽基质分析法步骤,检查AT-Ⅲ、PLG及APL活性。各组的结果均低于正常值,P<0.01,有非常显著差异。不过活性值仍高于60%。手术时间长短对活性无影响。因缺乏DIC的临床症状,故此种活性降低是手术创伤后早期机体凝血及纤溶系统的防卫反应。它与手术创伤的严重程度有关,而与手术时间长短无关。用这种方法监测大手术病人,对判断病情的发展和术后合并症有一定的实际意义。

Clinical criteria of blood coagulation and fibrinolysis at the early stage of 38 cases of severe burns had been examined. The results revealed that burn patients had their blood platelet count markedly reduced, prothrombin time and thrombin time prolonged and fibrinogen content and the positive rate of protamine paracoagulation test ( "3P" test ) and alcogel test increased, as com-pared with the normal controls, but the result of euglobulin lysis test did not change significantly.

本文通过检测分析38例大面积烧伤病人的BPC、PT、FN、3P、乙醇胶TT及ELT等指标的变化,认为严重烧伤,尤其是伴有广泛Ⅲ度伤的大面积烧伤病人普遍存在消耗性凝血和纤溶亢进状态,并有继发DIC的倾向。

Fifteen adult mongrel dogs have been inflicted by various degrees of blast injury. These animals were divided into four groups; mild, moderate, severe and extreme severe group. The results show that platelet count was significantly decreased ia all groups (P<0.03), PT, KPTT, TT and fibrinogen had no changes, ELT was shortened and 3P test was positive in all groups. The reversible hemoconcentratioa occurred in the early period of the mild group and severe group, the moderate group had no change, but irreversible hemoconcentratioa was seen ia the extreme severe group. It is concluded that the decreases ia platelet count and primary fibrinolysis after various degrees of blast injury are the characteristic changes of function _of_coagulation and fibrinolysis, but coagulation function itself was not changed.

用15条成年杂种犬在遭受不同程度的冲击伤后,研宄伤后立即、第1h、第6h和第24h的凝血和纤溶功能的变化,并和伤前结果作对照。测定项目也括血小板计数、纤维蛋白含量、凝血酶原时间、白陶土部分凝血活酶时间、凝血酶时间、优球蛋白溶解时间、3P试验和红细胞压积。根据犬肺部受伤的程度分为轻伤组、中伤组、重伤组和极重伤组。结果表明血小板数在各组均有明显意义的减少(P<0.05),PT、KPTT、TT和Fbg在各组均无变化,各组的ELT均缩短(P<0.05),3P试验均为阳性。红细胞压积在轻伤组和重伤组的早期有可逆性血液浓缩,中伤组无变化,在极重伤组则为不可逆性血液浓缩。结论认为犬受到不同程度的冲击伤后,凝血纤溶功能变化的特点是血小板数减少和发生原发性纤溶,而凝血功能本身无改变。
