481 cases (569 eyes )of corneal diseases who were admitted to the authors' hospital during 1985 to 1990 with completed records were analyzed. It was found that the inci dence ratio of male to female was about 2:1,and the ages was mostly(53. 6 % )from 21to 50 years. The chlef causes of corneal blindness was found to be corneal inflammatory diseases and injury. which indicated that attention should be paid to preventing and rapidly treating the illnesses mentioned above.

本文统计分析1985~1990年间我院眼科481例(569只眼)住院角膜病患者的病种分布和致盲原因、调查结果表明,发病性别男多于女(2:1),发病年龄以21~50岁为多,占53.6%,最常见及致盲率高的为角膜炎症和外伤,其中病毒性角膜炎最多(33.9%),细菌性角膜溃疡致盲率最高(42.2%),角膜外伤居第二。分析表明,今后工作的重点是预防和治疗角膜炎症和外伤。

Purpose:To evaluate objectively the clinical efficacy and safety of recombinant human epidermal growth factor(rhEGF)on enhancing corneal epithelial defect healing.Methods:A prospective,random study was carried out.The 153 corneal blindness cases (153 eyes)with limbal normality were treated by penetrating keratoplasty in which each graft measured 7 5mm in diameter and all donor epithelia were removed during operation.All the 153 cases were randomly divided into ecperimental groups(91 eyes)treated by rhEGF eyedyops,positive control group (31 cases)treated by Solcoseryl Eye Gel,and negative group (31 cases)treated by vehicle.The healing rates of corneal epithelium were observed postoperation.Results:Time until epithelium defect healing was significantly shorter with rhEGF compared with vehicle group (P<0 01)and Solcoseryl Eye Gel group (P<0 01).Conclusion:The results indicated that rhEGF improved effectively the healing of corneal epithelial defect.The clinical optimal safety concenration is 20μg/ml.

目的 :客观评估重组人表皮生长因子 (rh EGF)促进角膜上皮损伤修复的临床疗效及安全性。方法 :应用前瞻性评估研究方法 ,对无角膜缘干细胞异常的角膜盲 15 3例 15 3只眼施行穿透性角膜移植术。移植片直径统一为 7.5 mm,术中去除供体角膜上皮 ;将所有研究病例按随机双盲原则分试验组 (91眼 )、赋型剂组 (31眼 )及素高捷疗组 (31眼 )。术后分别观察移植片角膜上皮的愈合速率。结果 :各 rh EGF组术后移植片角膜上皮愈合时间比赋型剂组及素高捷疗组明显缩短 (t检验 ,P<0 .0 1)。结论 :rh EGF滴眼液能有效促进角膜上皮损伤的修复 ,其临床最佳安全浓度为 2 0μg/ml

OBJECTIVE:Corneal allograft is the most effective therapy for corneal blindness,but corneal graft rejection is still the primary cause of transplantation failure.How to prevent and treat the postoperative corneal graft rejection is always the emphasis and difficulty in the field of keratopathy research.Corneal graft rejection is a complicated reaction mainly led by T lymphocyte.As main co stimulatory molecules of T lymphocyte activation,B7 plays a very important role in the occurrence and treatment of corneal graft rejective reaction.DATA SOURCES:A computer based search of Pubmed and Medline for relevant studies to corneal transplantation and B7 molecule both published between January 1990 and October 2004 was performed using key words of “ keratoplasty,corneal allograft,corneal transplant,B7,CD80,CD86",with language of the articles limited to English.STUDY SELECTION:After the first trial for the articles,the experimental articles were adopted,and the articles about case report and experience summary were screened out.The full text of the rest articles was searched for to judge whether they were in accordance with the inclusion criteria.DATA EXTRACTION:Totally 31 experimental literatures related to corneal transplantation and B7 were collected,and 21 articles were accorded with the inclusion criteria.DATA SYNTHESIS:A total of 724 samples were included in the 21 experiments.The therapeutic programs using B7 antibody or CTLA 4Ig,etc.were experimented and evaluated.It was shown from the experiments that these therapeutic regimens had effectively inhibited or relieved corneal graft rejection at different degree.CONCLUSION:As co stimulatory molecules of T lymphocyte activation,B7 has gradually become a hot research field of corneal transplantation immunology in the treatment of corneal blindness.Researches show that corneal graft rejection induced by corneal transplantation can be relieved by the blockade to second signal of T cell activation,which results from inhibition of B7 co stimulatory molecules so as to provide a newly key point for the controlling the therapeutic effects of corneal transplantation.
