本文用心阻抗血流图观察了22例病人在静脉普鲁卡因复合麻醉下,硝普钠控制性降压期间血流动力学的变化。结果表明,降压期间MAP、TPR、TTI显著下降,SV和CO增加,HR明显增快。停用硝普钠后,MAP、TPR和TTI迅速升高,HR减慢,CO减少,而SV没显著变化。提示硝普钠降压中只要及时补充血容量,可以维持心脏的搏血功能。

Forty adult patients undergoing non-cardiac major surgery were divided into 2 groups. Group A (n=20) was anesthetized by balanced intravenous procaine anesthesia and guoup B(n=20) by epidural block. Blood Volume, Cortisol, aldosterone ,Glucose, Insulin ,TT_3 TT_4 ,rT_3 ,ATG and MCA levels in plasma were measured from 24 hours before operation to 48 hours after operation. The mechanism of these changes and clinical significances was discussed.

非体外循环大手术成年患者40例,分普鲁卡因静脉复合麻醉和连续性硬膜外麻醉两组(n=20),全面监测自术前24h至术后48h期间血容量、血浆醛固酮、葡萄糖、皮质醇、胰岛素、总T_3(T T_3),总T_4(T T_4),反式T_3(r T_3),甲状腺球蛋白抗体(ATG)、微粒体抗体(MCA)。讨论了测值变化机制及其临床意义。

The study was designed to use electron spin resonance(ESR)spectroscopy to determine oxygen free radicals (OFR)in plasma in 30 selective surgery patients with intravenous procaine anesthesia (IPA)and to study the eliminating effects of naloxone and methylene blue on OFR. The results showed that the plasma OFR and lipid peroxide (LPO) of patients were significantly increased at 120 min after IPA as compared with before the induction. Thirty minutes after naloxone(loμg/kg) and methylene blue IV administration,both indexes were significantly decreased as compared with before treatment. It was indicated that both drugs could directly inhibit lipid peroxidation of cells and have OFR-scavenging properties during IPA.

采用电子自旋共振(ESR)技术检测30例择期手术病人静脉普鲁卡因复合麻醉(IPA)期间血浆氧自由基(OFR)及血浆脂质过氧化物(LPO)含量变化,并分别观察了纳洛酮(Naloxone,NAL)、亚甲蓝的抗氧化作用。结果显示:IPA120min时血浆OFR、LPO较麻醉前显著升高。应用纳洛酮或亚甲蓝治疗后30分,OFR、LPO较治疗前显著降低。揭示两种药物能抑制细胞的脂质过氧化反应,对IPA期间产生的OFR具有清除作用。
