Three changeable regurgitant orifices (round, rectangular and triangular) were developed. The 3D data were acquired using a rotation scan strategy with the transducer mounted on a mechanical arm and rotated from O°to 180°under the control of a TomTec computer. Color Doppler flow imaging was performed using an lnterspec Apogee system. Results showed that the moderate correlation was found when 2D color Dopler maximal jet areas were compared with regurgitant vloume (r=0. 84, SEE=2. 3ml, P<0. 0001) and maximal flow rate (r=0. 81, SEE=2. 4ml, P<0.0001). The close correlations between the 3D reconstructed jet volumes and measured regurgitant flow volumes (ml/beat) and maxirnal regurgitant flow rates (1/min) were found for pooled data for all three orifices (r=0. 92, SEE=4. 0ml, P=0. 0001 and r=0. 88,SEE=4. 6ml, P=0. 0001, for fIow volume and maximal flow rate, respectively);a better relationship betweent the 3D reconstructed maximal regurgitant jet volumes and the parameters were found when calculated separately for each orifice (for regurgitant volumes circular, r=0. 96; rectangular, r=0. 99;triangular, r=0. 99). Conclusion: Although the 3D jet volumes are also affected by aliasing velocity, geometrical shape of orifice and the other limitations of color Doppler flow mapping. the resuIts of 3D are superior to conventional 2D color Depler method and 3D reconstruction could facilitate approaches for quantifying the regurgitant jet.

本研究采用体外血流模型实验评价三维(3D)超声重建的方法定量瓣膜返流的大小,并与二维(2D)彩色血流显像对照。实验中选择三种返流瓣口(圆形、矩形和三角形)。仪器使用InterspeApogee彩色多普勒超声仪,探头附着在一种机械臂上,在TomTec计算机控制下进行0°至180°的旋转扫描获得三维重建的数据。结果示:2D彩色多普勒血流显像的最大面积与实际返流容积和最大返流量呈中等度相关(r=0.84和0.81)。3D重建后的返流容积与实际测定的返流容积(ml/beat)和最大返流量(L/min)相关良好(r=0.92和0.88);当分别测定单瓣口重建的返流容积时,其结果与实际返流容积相关更好(圆形,r=0.96;矩形,r=0.99及三角形,r=0.99)。结论:尽管3D重建返流容积的大小同样受混叠速度、瓣口形状的影响,但3D重建与实际返流量对照研究的结果仍明显优于2D彩色血流面积法,说明3D重建是定量评价瓣膜反流较准确的方法。
