Objective To investigate the spatial and temporal regulation effect of VEGF on human fetal retinal vascularization and angiogenesis. Methods The posterior segmental retinas from 54 human fetuses of the 9th week to the 40th week were studied by immunohistochemistry staining for the expressions of VEGF and PCNA. Results 1.The distribution of VEGF expression was spiking and the peaks were during the 9th~13th and around the 26th week.2.PCNA immunoreactivity was localized in spindle cells and vascular endothelial cells.The expression level was fluctuated during the developmental process.The peaks were during the 9th~13th and around the 21st week.In these periods,the spindle cells kept proliferating and differentiating,and remodelled subsequently to form the inner side retinal vessels.From the 26th or 34th week,the PCNA immunoreactivity is fully expressed in the vascular endothelial cells of the inner and outer margin of inner nuclear layer(INL) and kept to full terms.3.Significant positive correlation were shown between the content of VEGF in the retina and that of PCNA in spindle cells and vascular endothelial cells(r=0.736,P<0.01). Conclusion VEGF was positively involved in modulating human fetal retinal vascularization and angiogenesis.

目的研究血管内皮生长因子(vascularendothelialgrowthfactor,VEGF)对人胚胎视网膜血管发生的调节作用。方法收集54例9~40周龄胎儿眼球后壁标本,免疫组织化学染色,光镜观察。结果①VEGF在视网膜的表达呈波峰式分布,高峰在9~13周及26周左右。②节细胞层的梭形细胞(血管内皮细胞前体细胞)、血管内皮细胞呈增殖细胞核抗原(proliferationcelnuclearantigen,PCNA)免疫反应阳性,水平波动,高峰在9~13周及21周前后,此期间梭形细胞不断增殖、分化形成内皮细胞索,经改建形成视网膜内层血管,26、34周起见内核层内、外缘血管内皮细胞呈PCNA免疫反应阳性,并保持至足月。③视网膜VEGF表达量与梭形细胞、血管内皮细胞PCNA表达量呈显著正相关(r=0.736,P<0.01)。结论VEGF对视网膜血管的发生有显著促进作用

Angiogenesis plays an important role in vascular diseases of the retina and it is a severe complication of many retinal diseases.Until recently,little was known about its pathogenesis.This article reviews the processes and factors that regulate fetal retinal vascular development.The retinal vascularization of human fetus comprises two processes:initially the inner sides of retinal vessels are developed from spindle cells,which then are developed into vascular plexus by differentiation,proliferation and remodelling;the outer sides of retinal vessel arise as cellular buds to form capillary networks.Many factors can modulate angiogenesis,while vascular endothelial growth factor and basic fibroblast growth factor are the stronger factors to promote the angiogenesis.

视网膜血管病变中新生血管的发生占有重要地位,是很多视网膜疾病的严重并发症,其发病机制尚未完全明了,综述了胚胎视网膜血管发生方式及其促进因素。人胚胎视网膜血管发生存在两个阶段,第一阶段为视网膜内层血管的发育,由梭形细胞增殖分化形成血管内皮细胞,再改建成血管,第二阶段是外层血管的发育,由已存在的内层血管以出芽方式形成。血管的生成受到多种因素的调节,促进血管新生的调节因子有很多,其中血管内皮生长因子,碱性成纤维细胞生长因子是到目前为止发现最强的促进血管新生的生长因子。

ObjectiveTo observe the fundus,angiographic and histological changes of monkey eyes in which human fetal retinal pigment epithelium(RPE) cells were transplanted into the subretinal spaces.MethodsSmall patches of RPE monolayer were dissected from human fetal eyes aged from 12~24 weeks and cultured in regular culture dishes;routine vitrectomy was performed and loaded patches was injected into the subretinal spaces of monkey eyes after producing a bleb detachment of the neural retina.The fundus and angiographic changes were followed up weekly in the first month post operation and then monthly.The eyes in which we found remarkable immunorejection and those of 6 month post operation were extracted.Photomicroscope and electromicroscope examination were taken.ResultsThese transplanted human fetal RPE patches could survive at least one month under the retina of monkey eyes and maintained intimate association with the host photoreceptors.Normal fluorescein angiogram could be seen.Remarkable immunorejection was initiated 12~24 weeks after transplantation in the monkey eyes.In fluorescein angiogram,marked hyperfluorescein was shown in the late phase.In histogram,large amount of monocytes infiltration could be found.The macular areas were more sensitive to the RPE xenografts than the other areas.ConclusionCultured human fetal RPE can be transplanted to the subretinal spaces of monkey eyes.The survival time of those xenografts is very limited if immuno suppressive agents are not administered.How to keep the transplanted patches in monolayer status and their original orientation,and how to suppress the immunorejection are the key points in a successful RPE transplantation.

目的 观察猴视网膜下腔植入人胚眼视网膜色素上皮 (RPE)后不同时段的眼底、荧光血管造影和组织学的改变。方法  12~ 2 4周的人胚眼色素上皮片和酶解后的浓缩色素上皮细胞悬液经一步法或二步法植入猴眼视网膜下腔。结果 猴眼发生排异反应的时间为 2~ 6月 ,猴眼黄斑区发生排异反应的比例高于黄斑周围区。结论 异种RPE视网膜下腔的移植在无免疫抑制剂的条件下 ,只能短期存活。
