It is influenced by several physiological and pathological factors and associated with several cardiac diseases. We studied repolarization duration variability (RDV) and dependence of RTm/RR among normal subjects of different age groups, as well as between normal subjects and subjects with cardiac diseases. It was found that RDV and dependence of RTm/RR are related to age. There are significant differences in the slope of RTm/RR, coherence, CV ratio and RDV between normal subjects and subjects with cardiac diseases. The RDV and dependence of RTm/RR can be employed in providing a more useful evaluation of cardiac disease and function in clinical care.

Cardiovascular disease is a growing problem recently. There has been a few attention in the study of ventricular repolarization duration variability. RDV is associated with some cardiac diseases. We studied vagal blockade effects on RDV in urethane-anesthetized rats. To study the relation between ventricular repolarization duration and autonomic nervous system in subjects, we changed the balance of autonomic nervous system by body posture. The HRV and RDV spectral analysis showed that HF peak was modulated by the frequency of controlled respiration. We also found a significant decrease in HFnu and increase in LF/HF in HRV and RDV during vagal blockade. There is a significant increase of LF/HF of subjects in HRV and RDV from supine to tilt, which is consistent with the state of autonomic nervous system. The result of our study is that the ventricular repolarization duration is dependent on the autonomic nervous system. The RDV analysis will be more helpful in the clinic. This approach will provide a more useful noninvasive evaluation of cardiac disease and function.

目前,随着心血管疾病日益增多,国外有少量研究者开始分析心定复极化时程变异(Repolarizationdurationvariability,RDV),某些心脏疾病会在心室复极化时程变异上会有所表现。我们用乌拉坦麻醉大白鼠,研究用阿托品阻断迷走神经前后,RDV的变化规律。通过正常人体位变化来改变人体自主神经系统的平衡状态,以研究心室复极化时程与自主神经系统的关系。发现大白鼠的HRV和RDV的谱分析的峰值频率都受到吸呼频率的调制。阻断迷走神经后,HRV和RDV的HFnu都显著减小,LF/HF都显著增加。人体倾斜时,HRV和RDV的LF/HF显著高于平躺时,这与体位变化时自主神经系统的变化是一致的。从动物实验和人体实验结果看到心室复极化时程受到自主神经系统的影响。随着心室复极化时程变异研究的不断深入,将使评价心脏疾病和功能的无创性方法更趋完善,必将在临床上发挥更大的作用。

Objective:To investigate ventricular repolarization duration variability (RDV) both in normal individuals and in patients with acute myocardial infarction(AMI). Methods:RDV analysis of 24-hour Holter tapes was performed in 30 patients and 30 normal individuals.After pre-processing of digitized 24-hour Holter ECGs,heart rate variability and repolarization duration variability were analysed. Results:①In normal individuals,the circadian variation existed significantly in ventricular repolarization(RD)?high frequency (HF) and low frequency (LF).②All values of RDV in AMI were significantly low compared with the controls (p<0.05),and circadian variation was disappeared.③In normal individuals,the frequency contents of the RDV and HRV are qualitatively very similar. Conclusion:RDV may provide an additional noninvasive tool to evaluate the neural mechanisms regulating ventricular repolarization.It may be predictive for malignant arrhythmias in patients after AMI.

目的 :研究正常人及急性心肌梗死患者的心室复极时间变异性 (RDV)。  方法 :对 30例急性心肌梗死患者 (急性心肌梗死组 )及 30例健康人 (正常对照组 ) ,采用 2 4小时动态心电图监测系统进行记录 ,同步分析RDV及心率变异性 (HRV )。  结果 :①正常对照组高频、低频显示出明显的昼夜节律性变化 ,夜间明显高于白昼 (P <0 0 5 )。②急性心肌梗死组患者的SDNN、SDANN、高频、低频均低于正常对照组 ,有显著差异 (P <0 0 5 ) ,且失去了昼夜间差异。③RDV的频谱分析呈现出与HRV相似的两个主要的频谱成分即高频和低频。高频在正常人RDV与HRV有较好的相关性 (γ=0 82 ,P <0 0 1) ,而在急性心肌梗死患者此相关性降低 (γ =0 6 1,P <0 0 5 )。  结论 :RDV提供了又一无创方法评价心室复极的自主神经调节机制 ,可对心肌梗死患者的恶性心律失常进行预测。
