The departments of burn surgery , urogenital surgery, neurosurgery, and orthopedics under the surgical system had the, highest prevalence of hospital cross infections and special attention should be paid to them in the prevention of nosocomial infections, staphylococcus aureus was the most prevalent pathogen and pseudomonas aeruginosa was the second. The Lower respiratory tract, the operative incisions, the burn wounds, and the urogenital tract were the sites most frequently involved. The risk factor of a patient was closely dependent on whether there was the occurrence of nosocomial infection or not. There was a tendency to a lowering of the prevalence rate of nosocomial infections along with the 5 times of general survey, which may result from the preventive measures adopted by the hospital authorities to strengthen the control on the problem.

为了了解某综合医院医院感染的基本情况,于1990年4月~1992年1月,每季度一次共作5次医院感染现患率调查,调查该院23个病房共3516例住院病人,发现医院感染病例270例,现患率7、68%。医院感染各科分布,以烧伤科、泌尿外科、脑外科、骨科等外科系统感染率最高,是预防医院感染的重点科室。引起医院感染的细菌以金黄色葡萄球菌为主,绿脓杆菌次之;感染部位以下呼吸道、手术伤口、烧伤创面以及泌尿道为主;有无危险因素与医院感染有显著关联。5次调查的现患率有下降趋势,这可能与领导重视、对医护人员的宣教,加强对医院感染的监控有关。

This paper presents the situation of nosocomialinfection in our hospital. 878 cases accounting for99.7% of total number were studied and the infectionrate was 5.13%. The highest rate in Othopeadic De-partment was 18.60%, followed by 13.24% in theWard of High Official. The rate higher in Surgery thanin Internal Medicine. The lower respiratory tract andsurgical incision were the most frequent infection sites.There was great difference between the patientssuceptible to infection and general patients.

本文为海南省人民医院医院感染的横断面研究——现患率调查。实查人数878例,现患率为5.13%。各科室现患率骨科最高(18.60%),其次是高干病房(13.24%),外科系统高于内科系统。感染部位主要为下呼吸道和术后伤口。有易感因素和施行侵入性操作病人感染率与普通病人相比,有显著性差异。

Objectives: To evaluate the present nosocomial infection(NI) in northeast area, and to provide basis for taking policy to control the nosocomial infection. Methods: Using the prevalence survey of the epidemiology, we have investegated simultaneously the present nosocomial infection in 17 hospitals in the northeast area for 4 days. Results: First, NI rate was 7.75% in 5 704 patients, among which respiratory infection has been mainly seen as 57.47%, then 15.38% urinary infection and 11.99% surgical infection....

Objectives: To evaluate the present nosocomial infection(NI) in northeast area, and to provide basis for taking policy to control the nosocomial infection. Methods: Using the prevalence survey of the epidemiology, we have investegated simultaneously the present nosocomial infection in 17 hospitals in the northeast area for 4 days. Results: First, NI rate was 7.75% in 5 704 patients, among which respiratory infection has been mainly seen as 57.47%, then 15.38% urinary infection and 11.99% surgical infection. Secondly, 24.37% of the patients who received invasive operation were suffered from NI. The relative dangerous rate(RR) was 4.2, a microbiology laboratory report was only available for 29.86% of NI patients; 96.83% NI patients have received antibiotic treatment in which 62.44% patients have been treated for≥8 days and 24.89% of them have received more than three kinds of antibiotic treatments. Conclusion: Respiratory infection was the major cause of the present NI in the northeast area. Invasive operation was an important factor causing NI. The clinicians should highly consider pathogen examination. Long lasting abusing of broad spectrum antibiotics should be avoided.

目的 :为了解东北地区医院感染的现状 ,为进一步制定医院感染监控措施提供依据。方法 :采用流行病学现患率调查方法 ,连续 4天对东北地区 17所医院同时进行调查。结果 :调查 5 70 4人 ,平均医院感染的现患率为 7.75 %。以呼吸道感染占首位 ( 5 7.4 7% ) ,其次为泌尿系 ( 15 .3 8% )、外科伤口 ( 11.99% ) ,接受侵入性操作者医院感染发生率 2 4 .3 7% ,相对危险度 ( RR)4 .2。医院感染患者病原学检查率仅占 2 9.86% ,抗生素使用率 96.83 % ,其中≥ 8天者占 62 .4 4 % ,三种以上联合用药≥ 8天者占 2 4 .89%。结论 :东北地区医院感染以呼吸道感染占首位 ,侵入性操作是造成医院感染的重要因素。应提高临床医师对病原学检查的自觉性 ,避免长期滥用广谱抗生素
