Objective To evaluate the potential of perfusion weighted imaging (PWI) in diagnosis and differential diagnosis among malignant glioma,metastatic tumor and abscess of brain. Methods PWI was performed in 31 patients to get signal intensity percentage time curves of perilesion and contralarteral symmetric region,which were used to calculate maximum signal reduction ratio (SRRmax) and relative cerebrum blood volume (rCBV). The ratio Q of SRRmax and rCBV in both ROIs was calculated. In addition,SRRmax and rCBV of the lesions were calculated. Results The Q SRR values were 1.88,0.31 and 0.95 in grade Ⅲ-Ⅳ gliomas,while Q rCBV values in metastatic tumors and brain abscess were 2.61,0.26 and 1.01 respectively. The difference in the Q values was statistically significant among three groups,and the values of lesions had no obvious discrepancy. Conclusion PWI is very useful in diagnosis and differential diagnosis among grade Ⅲ-Ⅳ gliomas,metastatic tumors and abscess of brain.

ObjectiveTo evaluate the role of dynamic contrast enhanced MR imaging with 0.5T in the diagnosis of benign prostatic hyperplasia (BPH) and prostate cancer. MethodsTotal 72 cases suspected with BPH or prostate cancer underwent plain and dynamic contrast enhanced MR imaging, and the characteristics of contrast were described by early phase enhancement parameters and time signal intensity curves (SI Time curves). SI Time curves were classified into 3 types according to their shapes: Type Ⅰ, steady enhancement; Type Ⅱ, plateau of signal intensity, and Type Ⅲ, washout of signal intensity. After operation, all of the specimens obtained were confirmed by histopathology. Results The early phase enhancement parameters of dynamic contrast enhanced MR imaging (onset time, maximum signal intensity and early phase enhancement rate) obtained from 32 cases of prostate cancer differed from those from 40 cases of BPH ( P <0.01, 0.05, 0.01 ), but there was a certain extent of overlap. The benign and malignant characteristics of intermediate and late periods of post contrast which were described by the SI Time curves were as follows: Type Ⅰ most appeared in cases of BPH, Type Ⅱ could be found both in BPH and prostate cancer, whereas the SI Time curves of prostate cancer were a majority of Type Ⅲ ( P < 0.001 ). ConclusionDynamic contrast enhanced MR imaging has certain value in differential diagnosis of prostate cancer and BPH.

目的 探讨 0 .5T条件下MRI动态增强扫描对良性前列腺增生 (BPH)与前列腺癌的诊断与鉴别诊断价值。方法 对BPH 40例 ,前列腺癌 3 2例患者常规行MRI平扫和动态增强扫描 ,观察病灶动态增强特征并绘制时间信号强度曲线 (SI Time曲线 )。结果 前列腺癌MRI动态增强的开始时间、最大信号强度及早期强化率与BPH不同 (P分别 <0 .0 1、0 .0 5、0 .0 1)。SI Time曲线Ⅰ型多见于BPH ,Ⅱ型既可见于前列腺癌又可见于BPH ,前列腺癌以Ⅲ型为主 (P <0 .0 0 1)。结论  0 .5T条件下MRI动态增强早期强化参数及SI Time曲线类型有助于BPH与前列腺癌的诊断与鉴别诊断。
