本文用药物和手术方法治疗了32例单侧声带麻痹。发现:6个月后大部分病例出现自身代偿,对未能代偿的病例进行手术治疗,行声带内移术6例,声带内注射术1例,神经肌蒂移植术2例,发音质量获得不同程度的改善。神经肌蒂移植术是治疗声带麻痹的理想办法。电起搏动态治疗单侧声带麻痹有待进一步研究。

分析42例声带麻痹病例,特发性麻痹最多,甲状腺切除和外伤是重要的致病因素。大部分单侧声带麻痹者在6个月后出现发声代偿,未有代偿者以声带内注射术、声带内移术治疗可改善发音质量,电起搏动态治疗单侧声带麻痹有待进一步研究。双侧声带麻痹采用声带外移术治疗,Woodman术发音、呼吸效果较好,一侧声带切除和T型管扩张可获得安全的气道,神经肌蒂移植术目前尚无肯定的效果。

In order to study the reactivity of denervated posterior cricoarytenoid muscle of different time to electrical stimulation and explore the possibility of clinical application of laryngeal pacing, 14 dogs were examined at 0,2 weeks, 1, 2, 4, 8, 12 months after recurrent laryngeal nerve was damaged by a 2 - cm neurectomy, or dissection followed by resuturing. The results showed that the minimum threshold level to induce vocal cord abduction was 2 V at time after nerve injury, and the threshold level reached the maximum (7 or 10 V)at 2 weeks after nerve injury , then decrease with time, and recover initial level in each case of nerve resuturing group or keep about 6 V in neurectomy group 4 months after nerve injury. It was concluded that electrical pacing was effective to restore function of denervated posterior cricoarytenoid muscle.

为了研究电起搏器对失神经后不同时期环杓后肌的作用,我们选用14只狗分为喉返神经切断及吻合两组,分别在手术后即刻、2周、1、2、4、8、12个月进行观察,结果发现在神经切断及吻合即刻环杓后肌电刺激强度阈值较低(2伏),2周后达最高峰(7~10伏),4个月后,神经吻合组恢复到最初水平,神经切断组维持在6伏左右,并发现随病程延长环杓后肌萎缩加重.因此,电起搏器一定刺激强度在一定时间内对恢复萎缩环杓后肌的功能是有效的.
