Objective To find out the environmental epidemic characteristics of coal burning fluorosis in China, to determine the safety threshold of coal fluoride content, and to provide scientific basis for environmental risk assessment of fluoride.Methods Local investigation integrated with data analysis. Results Coal burning fluorosis was closely related to geochemica parameters of local environment, but the determinant factor of this endemic disease was the existence of high fluoride coal. There existed a highly linear correlation between the incidence of dental fluorosis and the content of coal fluoride in disease affected areas. And the safety threshold of coal fluoride content was determined as 250 mg/kg.Conclusions The coal fluoride content should be an major indicator to evaluate the quality of coal resource, for the patient caused by coal burning fluorosis should be detected early, diagnosed early and operated early,the prevention and treatment methods should integrate disease curing with poverty curing.

目的 查明我国燃煤污染型氟中毒的环境流行特征 ,确定煤炭氟的安全阈值 ,为氟的环境风险评价提供科学依据。方法 现场调查和资料分析相结合。结果 燃煤污染型氟中毒的流行与地球化学环境密切相关 ,但高氟煤炭的存在是其流行的决定因素。病区居民氟斑牙患病率与煤炭氟含量之间存在线性函数关系 (r =0 .943 2 ,P <0 .0 1) ,去掉 5 %的氟斑牙诊断误差和模糊判定因素等 ,煤炭氟的安全阈值为 2 5 0 mg/kg。结论 煤炭氟含量应作为煤炭资源开采与否的一个指标。对于燃煤污染型氟中毒病人应早发现、早诊断、早治疗 ,其防治应把治病与治穷结合起来

Abastract: Objective: To study women's menopause status and its affective factor in endemic fluorosis area. Methods: By environmental epidemic survey. Results: Certain fluoride content in drinking water can meet a later menopause age and the living longer the later of menopause age in endemic fluorosis area. The distribution of menopause age in 38~60 women showed two kurtosis in this area. The prevalence rate of dysmenorrhea was lower than control group (relative risk (RR)=0.82, 95% confidence interval: 0.68 to 0.98 ). The menopause age was not significant between smoking group (P>0.05). Menstruation disorder rate was higher in fluoride group (P<0.05). Conclusion: Certaion fluoride in drinking water may connect with a getting menopause age later and reduce some disease rate in endemic fluorosis area. As having been reduced fluoride in water, women's genital toxicity and fecundity can be changed.

目的:探讨地方性氟中毒病区氟对更年期妇女的绝经年龄的影响及其影响因素。方法 :采用环境卫生学调查方法对新疆奎屯垦区 2 5 36名 38~ 6 0岁妇女进行了调查研究。结果:氟对更年期妇女绝经年龄的影响呈现双峰趋势 ,提示氟对妇女的生殖内分泌功能的作用为双重的剂量反应效应。与对照组 (水氟浓度≤ 1mg/ L )相比 ,高氟组妇女绝经年龄延迟 (P <0 .0 5 ) ,痛经曾患率低 (RR=0 .82 ,95 % ,CI:0 .6 8~ 0 .98)。其中低度高氟组 (水氟浓度 1~ 3mg/ L)绝经概率变化速率快于对照组 (P <0 .0 5 )。随着在高氟区居住年限的延长 ,妇女绝经年龄后移更为明显 (P <0 .0 5 )。 结论 :适量的氟 (水氟浓度≤ 1m g/ L)有利于机体健康 ,而高氟可使高氟区妇女更年期延迟 ,并可减少某些疾病的发生
