Objective In order to investigate the immediate hematologic changes in local cavity of hemangioma induced by Nd∶YAG laser interstitial irradiation. Methods Six cases of hemangioma were selected which was suitable for Nd∶YAG laser interstitial irradiation and for enough quantity(10 ml) of blood sample obtainable. They were interstitially irradiated by Nd∶YAG laser. Results There were significant changes in various hematologic and hemorheologic parameters. Erythrocyte, leucocyte, platelet and hemoglobin were all decreased (P<0 05). The whole blood viscosity (shear rate 190/s, 40/s, 5 75/s), plasma viscoity, hematocrit value, erythrocyte rigidity index, erythrocyte aggregation index, whole blood reduced viscosity and low shear rate blood relative viscosity were increased (P<0 05). There were no notable changes in the hematologic, hemorheologic and immunologic parameters (P>0 05). The hematologic and enzymologic parameters: ALT, AST and LDH were significantly increased(P<0 05). CPK and ALP were not notablly changed (P>0 05). Conclusions The blood viscosity in local cavity of hemangioma was increased after interstitial Nd∶YAG laser irradiation. Attention should be paid to the possibility of thrombosis. But long period observation did not confirm this phenomenon. So we would believe that this is a temporary danger and does not exert long term side effect on the body.

目的 研究Nd∶YAG激光组织间照射血管瘤后即刻瘤腔内血液学的变化。方法 收集适合Nd∶YAG激光组织间照射治疗且照射前后可自瘤腔内各抽取10ml血液的血管瘤病人6例,以Nd∶YAG激光行血管瘤组织间照射,照射功率密度为531×103Wcm2,能量密度为797×104Jcm2。激光照射前及照射后即刻取血进行血液细胞学、生化学、酶学、免疫学、流变学等共24项指标检测。结果 激光照射后即刻瘤腔内血液红细胞、白细胞、血小板数及血红蛋白含量下降(均P<005),丙氨酸转氨酶、天冬氨酸转氨酶、乳酸脱氢酶、全血粘度(切变率190s、46s、575s)、血浆粘度、红细胞比积、红细胞刚性指数、红细胞聚集指数、血液还原粘度、低切变率下血液相对粘度升高(均P<005),血液生化学、免疫学各指标无明显变化。结论 激光组织间照射后血液粘滞度升高,因此要注意防止血栓的形成。

Objective To explore the optimal heating power, heating time, effects and mechanisms of laser interstitial thermotherapy (LITT) for treatment of glioma. Methods The Walker glioma cells were subcutaneously transplanted into the rear legs of rats, the diameters of tumors reached 1.4 - 2. 0 cm 10 days after transplantation. The neodymium yttrium aluminium garnet ( Nd-YAG) laser was applied for interstitial thermotherapy under low power of 2 W, 5 W and 10 W respectively, and for heating time of 800 s, 800 s and 600 s respectively, meanwhile the control group was established. The changes of diameter, pathology and ultrastructure of tumor were observed. Results No obvious changes of tumor diameter were found between each treatment group and control group on day 3 and day 6 after LITT ( P > 0. 05) , however the tumor diameters of treatment groups were all less than those of control group on day 9 and day 12 after LITT (P < 0. 01). The carbonization of center tissue, denaturation and necrosis of outer layer tissue were found in treatment groups. The ultrastructures of tumor cell in treatment groups changed markedly too. Conclusion The LITT with low power Nd-YAG laser can inhibit the growth of glioma obviously, which having excellent application prospect.

目的 探讨激光间质热疗(LITT)治疗胶质瘤的最适热功率、照射时间、热效应及作用机制。方法 将Walker胶质瘤细胞接种于大鼠后肢皮下,接种10d后瘤体直径达1.4~2.0cm。分别用钕钇铝石榴石(Nd-YAG)激光在2W、5W、10W三种低功率下进行间质热疗,激光作用时间分别为800s、800s和600s,并设立对照组。观察肿瘤直径、肿瘤病理组织学及超微结构的改变。结果LITT后3d及6d各治疗组与对照组肿瘤直径无显著差异(P>0.05);LITT后9d及12d各治疗组肿瘤直径明显小于对照组(P<0.01)。治疗组肿瘤中心炭化,外围变性、坏死。治疗组肿瘤细胞超微结构也发生明显变化。结论 低功率Nd-YAG激光LITT可明显抑制胶质瘤的生长,具有良好的应用前景。

Objective: To study the method and long-term curative effect of diode laser interstitial heat therapy for pituitary tumor by transnasal-sphenoidal approach conducted by CT or X ray. Methods: 805nm diode laser, output power of which is 25W, optical fiber is conducted by transnasal-sphenoidal approach through CT or X ray to have heat therapy. Results: Symptoms get better for all 3 patients, and endocrine indexs are normal. 1 patient has no pituitary tumor 1 year after operation, and 1 patient is made craniotomy 2 years after operation. Conclusion: This methods have advantage of simple manipulation, accurate location, slight injury, less complication, less painful and economics.

目的 :观察与探讨经鼻蝶穿刺垂体瘤半导体激光间质内热疗术的方法与长期疗效。方法 :采用 80 5nm半导体激光 ,输出功率 2 5W ,CT或X线引导下经鼻蝶穿刺将光导纤维导入垂体瘤中心分次热凝。结果 :3例术后症状皆改善 ,内分泌指标正常。 1例术后 1年肿瘤消失 ,1例术后 2年开颅手术。结论 :本术式具有操作简单、方便快捷、定位准确、损伤轻、病人痛苦少恢复快、并发症少、经济安全等特点
