Objective\ To investigate the influence of combined immunosuppressants on CD62L and CD11a expression of lymphocytes from peripheral blood in renal allograft recipients and implication. Methods\ The dynamic changes in CD62L and CD11a levels in lymphocytes in different time points were examined by using mAb FACS immunofluorescence technique in 10 renal allograft recipients in stable course (stable group) and 10 with acute rejection (rejection group) after operation. The recipients were subjected to triple drug therapy and methylprednisolone (MP) was used for anti rejection treatment and in acute stage and antithymocyte lymphocyte globulin (ATG) was introduced simultaneously in the early stage (within 10 days). Results\ The expression of CD11a in lymphocytes was increased after administration of immunosuppressants in stable group, reaching the peak at 9th day [(22.9±8.9), P <0.01], and returned after stopping ATG, but significantly lower than in the preoperative group. No significant difference in CD62L between the suppressants treated group and preoperative groups was found. In rejection group, both CD62L and CD11a expression was obviously increased at the 5th to 7th day of rejection, and CD11a was rapidly decreased, but CD62L showed no change.Conclusion\ The immunosuppressants (especially MP) can suppress the expression of CD11a in lymphocytes evidently. ATG may play an important role in maintaining lower level of CD11a and inducing the immunologic tolerance.

目的 探讨联合免疫抑制剂对肾移植受者外周血淋巴细胞表面淋巴细胞功能相关抗原 1(CD11a)和L 选择素 (CD6 2L)表达的作用。方法 利用单克隆抗体 流式细胞仪荧光免疫技术 ,动态观察 10例肾移植稳定病人 (稳定组 )和 10例排斥反应病人 (排斥组 )淋巴细胞CD6 2L、CD11a表达 ;急性期及排斥反应时予甲基波尼松龙 (MP) ,早期辅以抗胸腺淋巴细胞球蛋白 (ATG)。结果 稳定组应用免疫抑制剂后CD11a表达降低 ,第 9d最明显 [(2 2 .9± 8.9) % ,P <0 .0 1],停用ATG后回升 ,但仍明显低于术前组 ,CD6 2L与术前组差异无显著性 (P >0 .0 5 )。排斥组 5~ 7d排斥反应时二者表达均明显升高 ,给予MP后CD11a迅速下降 ,而CD6 2L无变化。结论 联合免疫抑制剂能明显抑制CD11a抗原表达 ;ATG对协同CD11a低水平表达和诱导免疫耐受起重要作用。

Objective To investigate a clinically perspective way of inducing transplantation tolerance. Methods Kidney transplantation model was established using Lewis (RT1 l) rats as recipients and DA (RT1 a) rats as donor. Objective To investigate a clinically perspective way of inducing transplantation tolerance. Methods Kidney transplantation model was established using Lewis (RT1 l) rats as recipients and DA (RT1 a) rats as donor. Before transplantation the recipient rats were conditioned with anti lymphothyte serum (ALS) injected for three times, followed by 10 8 donor bone marrow cells transfusion (BMT) and injection of 100 mg/kg cyclophosphamide (CP), survival time of kidney allografts, mixed lymphocyte reaction (MLR) and delayed type hypersensitivity (DTH) of recipient to donor spleen cells were observed and studied. Results 5 of the 7 kidney allografts survived 73 to 90 days without sign of rejection. The donor specific MLR and DTH were suppressed. Conclusions Induction of transplantation tolerance is a prospective way for the prevention of allograft rejection.

目的 探讨具有临床应用前景的移植耐受诱导方法。 方法 以Lewis大鼠和DA大鼠分别作为肾脏移植的受体和供体 ,采用注射抗淋巴细胞血清 (ALS)、输入供体骨髓细胞 (BMT)和环磷酰胺 (Cp)注射方法进行移植耐受诱导 ,观察肾脏移植物的存活情况及受体对供体细胞抗原免疫应答改变。 结果 经耐受诱导的大鼠肾脏移植物存活时间明显延长 ,7只鼠中 5只移植物存活 73~90d时仍无排斥反应迹象 ,混合淋巴细胞反应及诱导迟发型超敏反应表现为供体特异性降低。 结论 诱导免疫耐受预防肾脏移植排斥反应具有重要的临床意义 ,此方法有一定的临床应用前景。

目的 探索供者骨髓细胞受者门静脉输注诱导移植肾免疫耐受的临床应用方法。方法  8例亲属供肾肾移植受者 (实验组 ) ,移植术前经门静脉注射供者的骨髓细胞 ,进行移植后的临床效果观察 ,移植肾肾内免疫状态分析 ,尿细胞流式细胞分析和细胞因子测定 ,结果与 6例亲属供肾肾移植病人 (对照组 )进行比较。结果 实验组在临床上和移植肾免疫状态上均表现出免疫低反应性 ,IL- 10 (Interleukin- 10 )表达较高 ,TNF- a(Tumor necrosis factor- α)表达较低。结论 我们认为 ,将供者骨髓细胞进行受者的门静脉输注是一种可行的诱导免疫耐受的方法 ,可以诱导产生临床肾移植的免疫低反应
